麦默通微创旋切术治疗良性乳腺肿块的临床效果

Clinical Effect of Mammotome Minimally Invasive Atherectomy in the Treatment of Benign Breast Masses

目的:探讨麦默通微创旋切术治疗良性乳腺肿块的临床效果。方法:选取2018年1月—2021年1月新疆生产建设兵团第三师总医院喀什院区收治的60例良性乳腺肿块患者为研究对象。根据随机数表法将其分为对照组和研究组,各30例。对照组给予常规乳房肿块切除术,研究组给予麦默通微创旋切术。比较两组围手术期指标,术前、术后3 d血管内皮生长因子(vascular endothelial growth factor,VEGF)水平,乳房外观满意度及并发症。结果:研究组手术时间、总住院时间均短于对照组,术中出血量少于对照组,术后1 d视觉模拟评分法(visual analogue scale,VAS)评分低于对照组,差异有统计学意义(P<0.05)。两组术后3 d VEGF水平均低于术前,且研究组VEGF水平低于对照组,差异有统计学意义(P<0.05)。研究组乳房外观总满意度高于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:良性乳腺肿块患者采用麦默通微创旋切术治疗可缩短手术时间,减少术中出血量,减轻术后疼痛程度,患者术后VEGF水平更低、乳房外观满意度高,利于早期康复。

Objective:To investigate the clinical effect of Mammotome minimally invasive atherectomy in the treatment of benign breast masses.Method:Sixty patients with benign breast mass admitted to Kashgar Hospital of the Third Division of Xinjiang Production and Construction Corps from January 2018 to January 2021 were selected as the objects.According to the method of random number table,they were divided into control group and study group,30 cases in each group.The control group was given conventional breast mass resection,and the study group was given Mammotome minimally invasive atherectomy.Perioperative indexes,vascular endothelial growth factor(VEGF)levels before and 3 d after surgery,breast appearance satisfaction and complications were compared between the two groups.Result:The operation time and total hospital stay of the study group were shorter than those of the control group,the amount of intraoperative blood loss was less than that of the control group,and the visual analogue scale(VAS)score at 1 d after surgery was lower than that of the control group,the differences were statistically significant(P<0.05).The levels of VEGF at 3 d after surgery were lower in both groups than those before surgery,and the level of VEGF in study group was lower than that in control group,the differences were statistically significant(P<0.05).The total satisfaction of breast appearance in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:For patients with benign breast masses,Mammotome minimally invasive atherectomy treatment can shorten the operation time,reduce the amount of intraoperative blood loss,and reduce the degree of postoperative pain.The postoperative VEGF level of patients is lower,and the satisfaction of breast appearance is higher,which is conducive to early rehabilitation.
