电针穴位刺激对2型糖尿病周围神经病变性疼痛患者炎性细胞因子的影响 被引量:1

Effect of Electroacupuncture Point Stimulation on Inflammatory Cytokines in Patients with Type 2 Diabetes Mellitus Peripheral Neuropathic Pain

摘要 目的探讨电针穴位刺激对2型糖尿病周围神经病变性疼痛患者的治疗效果。方法方便选择2019年1月—2022年12月在福建中医药大学附属第二人民医院接受治疗的120例2型糖尿病周围神经病变性疼痛患者,将其分为对照组(糖尿病基础治疗方案)和观察组(电针穴位刺激)。比较两组治疗10 d后的视觉类比量表(VAS)评分、正中神经、尺神经和腓总神经的感觉传导速度、炎性细胞因子和临床疗效。结果观察组治疗后的VAS评分为(3.35±0.51)分,显著低于对照组的(5.52±0.65)分,差异有统计学意义(t=20.345,P<0.05),观察组治疗后的血清TNF-α、IL-6和IL-1β均显著低于对照组,差异有统计学意义(P<0.05);正中神经、尺神经和腓总神经的感觉传导速度、总有效率显著高于对照组,差异有统计学意义(P<0.05)。结论电针穴位刺激可能通过抑制2型糖尿病周围神经病变性疼痛患者的炎症反应而达到镇痛、提高感觉神经传导速度的临床目标。 Objective To study the therapeutic effect of electroacupuncture acupoint stimulation on 60 patients with type 2 diabetic peripheral neuropathic pain.Methods 120 patients with type 2 diabetic peripheral neuropathy pain treated in the Second People′s Hospital Affiliated to Fujian University of Traditional Chinese Medicine between January 2019 and December 2022 were conveniently selected and divided into the control group(basic treatment plan for diabetes)and the observation group(Electroacupuncture point stimulation).The Visual Analogue Scale(VAS)score,sensory conduction velocity of median nerve,ulnar nerve and common peroneal nerve,inflammatory cytokines and clinical efficacy were compared between the two groups after 10 days of treatment.Results The VAS score of the observation group after treatment was(3.35±0.51)points,which was significantly lower than that in the control group(5.52±0.65)points,and the difference was statistically significant(t=20.345,P<0.05).The serum TNF IL-6 and IL-1βin the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).The sensory conduction velocity and total effective rate of the median nerve,ulnar nerve and common peroneal nerve were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Electroacupuncture acupoint stimulation may achieve the clinical goals of analgesia and improvement of sensory nerve conduction velocity by inhibiting the inflammatory response in patients with type 2 diabetic peripheral neuropathic pain.
