TeamSTEPPS对COPD患者院外用药知信行及用药差错的影响

Effect of TeamSTEPPS on KAP of Out-of-hospital Medication and Medication Errors in COPD Patients

目的:探讨提高医疗质量和患者安全性的团队策略与工具包(TeamSTEPPS)模式在慢性阻塞性肺疾病(COPD)患者院外用药指导中应用价值。方法:将2021年3月—2022年4月于我院诊治的COPD患者86例纳入研究,按随机抽签法分为试验组和对照组,每组43例。对照组实施常规院外用药指导,试验组实施基于TeamSTEPPS模式的院外用药指导。比较两组用药知信行状况、用药差错发生情况及病情程度[COPD评估测试量表(CAT)评分、改良呼吸困难评估量表(mMRC)评分]。结果:干预后,试验组用药认知、用药信念、用药行为评分高于对照组(P<0.05);试验组随访期间用药剂量差错、时间差错、剂型差错、漏服及自行停药发生率低于对照组(P<0.05);试验组CAT评分、mMRC评分低于对照组(P<0.05)。结论:TeamSTEPPS模式能有效改善COPD患者院外用药知信行状况,减少用药差错,利于病情控制,具有较高的临床应用价值。

Objective:To explore the application value of the team strategies and tools to enhance performance and patient safety(TeamSTEPPS)mode in out of hospital medication guidance for patients with chronic obstructive pulmonary disease(COPD).Methods:A total of 86 patients with COPD diagnosed and treated in our hospital from March 2021 to April 2022 were included in the study.They were divided into experimental group and control group by random lottery method,with 43 cases in each group.The control group implemented routine out-of-hospital medication guidance,and the experimental group implemented out-of-hospital medication guidance based on TeamSTEPPS model.The status of medication knowledge,belief and behavior,the occurrence of medication errors and the severity of the disease[COPD Assessment Test Scale(CAT)score,modified Medical Research Council Scale(mMRC)score]were compared between the two groups.Results:After intervention,the scores of medication cognition,medication belief and medication behavior in the experimental group were higher than those in the control group(P<0.05).During the follow-up period,the incidence of dosage error,time error,dosage form error,omission and self-withdrawal in the experimental group was lower than that in the control group(P<0.05).The CAT score and mMRC score of the experimental group were lower than those of the control group(P<0.05).Conclusion:TeamSTEPPS model can effectively improve the medication knowledge,belief and practice outside the hospital in patients with COPD,reduce medication errors,and facilitate disease control,which has high clinical promotion value.
