盐酸洛美沙星滴耳液联合阿莫西林克拉维酸钾治疗急性化脓性中耳炎的效果研究

Effect of Lomefloxacin Hydrochloride Ear Drops Combined with Amoxicillin and Clavulanate Potassium in the Treatment of Acute Suppurative Otitis Media

目的:研究盐酸洛美沙星滴耳液联合阿莫西林克拉维酸钾对急性化脓性中耳炎的效果及对患者血清miR-203a、炎症介质水平的影响。方法:选择2019年1月—2021年12月河南省焦作孟州市中医院收治的80例急性化脓性中耳炎患者,采用抽签法随机分为对照组和观察组,每组40例。对照组单独口服阿莫西林克拉维酸钾,观察组联合使用盐酸洛美沙星滴耳液。比较两组临床疗效、血清指标[血清miR-203a、白细胞介素-2(IL-2)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-4(IL-4)水平、外周血CD4^(+)T细胞和CD8^(+)T细胞]。结果:观察组有效率为97.50%,高于对照组的80.00%(P<0.05);观察组血清miR-203a、IL-2、TNF-α、IL-6和IL-4水平低于对照组(P<0.05);观察组外周血CD4^(+)、CD4^(+)/CD8^(+)和CD8^(+)优于对照组(P<0.05)。结论:盐酸洛美沙星滴耳液联合阿莫西林克拉维酸钾对急性化脓性中耳炎的效果较佳,能明显降低血清miR-203a和炎症介质水平,值得应用推广。

Objective:To study the effect of lomefloxacin hydrochloride ear drops combined with amoxicillin and clavulanate potassium on acute suppurative otitis media and its influence on serum miR-203a and inflammatory mediators.Methods:A total of 80 patients with acute suppurative otitis media admitted to Jiaozuo Mengzhou Hospital of Traditional Chinese Medicine from January 2019 to December 2021 were randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with amoxicillin and clavulanate potassium alone,and the observation group was treated with lomefloxacin hydrochloride ear drops.The clinical efficacy,serum indexes[serum levels of miR-203a,IL-2,TNF-α,IL-6,IL-4,peripheral blood CD4^(+)T cells and CD8^(+)T cells]were compared between the two groups.Results:The effective rate of the observation group was 97.50%,which was higher than 80.00%of the control group(P<0.05).The serum levels of miR-203a,IL-2,TNF-α,IL-6 and IL-4 in the observation group were lower than those in the control group(P<0.05).The peripheral blood CD4^(+),CD4^(+)/CD8^(+)and CD8^(+)in the observation group were better than those in the control group(P<0.05).Conclusion:Lomefloxacin hydrochloride ear drops combined with amoxicillin and clavulanate potassium have better effect on acute suppurative otitis media,which can significantly reduce serum miR-203a and inflammatory mediators,and is worthy of promotion.
