揿针配合PNF技术治疗偏瘫肩痛的效果及对疼痛水平、上肢功能、炎症因子表达的影响

Effect of Pressing Acupuncture Combined with PNF Technique on Hemiplegic Shoulder Pain and its Effects on Pain Level,Upper Limb Function and Inflammatory Factor Expression

目的:探讨揿针配合PNF技术治疗偏瘫肩痛的效果及对疼痛水平、上肢功能、炎症因子表达的影响。方法:选取156例于2021年1月-2022年2月丰城市人民医院收治的104例脑卒中偏瘫肩痛患者,按随机数字表法分为揿针组、PNF组和联合治疗组,各52例,比较三组患者治疗前、治疗1、2个月后肩关节疼痛水平(VAS评分)和上肢功能评分(FMA评分),比较三组患者治疗前后炎症因子水平,治疗前、治疗2个月后生活质量评分和临床治疗效果。结果:治疗前三组患者VAS评分、FMA评分比较(P>0.05),治疗1、2个月三组患者VAS评分下降,FMA评分升高(P<0.05),联合治疗组VAS评分低于揿针组、PNF组(P<0.05),FMA评分高于揿针组、PNF组(P<0.05)。治疗前三组患者IL-4、IL-6和TNF-α水平比较差异无统计学意义(P>0.05),治疗1、2个月后三组患者IL-4、IL-6和TNF-α水平均下降(P<0.05),联合治疗组IL-4、IL-6和TNF-α水平低于揿针组、PNF组(P<0.05)。治疗前三组患者生活质量评分比较差异无统计学意义(P>0.05),治疗后联合治疗组生活质量各项评分均高于揿针组、PNF组(P<0.05)。联合治疗组临床治疗有效率92.31%高于揿针组的75.00%和PNF组的76.92%(P<0.05)。结论:揿针配合PNF技术治疗偏瘫肩痛可以有效缓解患者肩部疼痛、改善患者上肢运动功能,减轻炎症反应并改善患者生活质量,临床效果较好。

Objective:To investigate the effect of snap needle combined with PNF technique in the treatment of hemiple-gic shoulder pain and the influence of pain level,upper limb function and inflammatory factor expression.Methods:156 patients with hemiplegic shoulder pain after stroke admitted to our hospital from January 2021 to February 2022 were selected and divided into snap needle group,PNF group and combined treatment group according to the random number table method,52 cases in each group.The shoulder pain level(VAS score)and upper limb function score(FMA score)of the three groups were compared before treatment,1 month and 2 months after treatment.The levels of inflammatory factors before and after treatment,quality of life scores before and 2 months after treatment and clinical treatment effects were compared among the three groups.Results:Before treatment,the VAS score and FMA score of the three groups were compared(P>0.05).After 1 and 2 months of treatment,the VAS score of the three groups was de-creased,and the FMA score was increased(P<0.05).The VAS score of the combined treatment group was lower than that of the press-needle group and PNF group(P<o.05),and the FMA score was higher than that of the press-needle group and PNF group(P<0.05).The levels of IL-4,IL-6 and TNF-αin the three groups before treatment were com-pared(P>0.05),and the levels of IL-4,IL-6 and TNF-αin the three groups after 1 and 2 months of treatment were decreased(P<0.05).The levels of IL-4,IL-6 and TNF-αin the combined treatment group were lower than those in the press-needle group and PNF group(P<0.05).The quality of life scores of the three groups were compared before treatment(P>0.05).After treatment,the quality of life scores of the combined treatment group were higher than those of the snap needle group and PNF group(P<o.05).The effective rate of combined treatment group was 92.31%(48/52),which was higher than that of snap-needle group 75.00%(39/52)and PNF group 76.92%(40/52)(P<0.05).Conclusion:The treatment of hemiplegic shoulder pain with snap needle combined with PNF technology can effectively relieve shoulder pain,improve upper limb motor function,reduce inflammation and improve the quality of life of patients,and the clinical effect is good.
