育龄女性子宫内膜息肉发病特征及宫腔镜术后自然妊娠失败的危险因素分析

Risk factors for recurrence of endometrial polyp in women of childbearing age treated by hysteroscopic endometrial polyectomy

目的探讨育龄女性子宫内膜息肉(EPs)的发病特征及宫腔镜子宫内膜息肉切除术(TCRP)自然妊娠失败的危险因素。方法选取2018年12月至2020年12月于浙江省杭州市临安区中医院妇科就诊的214例EPs患者,随访时间妊娠失败或满24个月为止。随访时间截止后根据是否自然妊娠失败,将研究对象分为失败组(24例)和成功组(190例),采用单因素和Logistic回归分析EPs患者TCRP术后自然妊娠失败的独立危险因素,建立列线图预测模型并进行验证。结果年龄>35岁、HRT治疗史、高血压、子宫内膜炎、术后妊娠均为EPs自然妊娠失败的高危因素。列线图模型拟合效果良好,且模型验证后显示预测能力较强。结论分析育龄女性EPs的发病特征及宫腔镜术后自然妊娠失败的危险因素,有助于提高患者预后水平。

Objective To explore the incidence characteristics of endometrial polyps(EPs)in women of reproductive age and the risk factors of natural pregnancy failure by hysteroscopic endometrial polypectomy(TCRP).Methods A total of 214 patients with EPs who visited the gynecology department of Lin’an District Hospital of Traditional Chinese Medicine in Hangzhou,Zhejiang province from December 2018 to December 2020 were selected,and the follow-up time was up to 24 months after pregnancy failure.After the follow-up cut-off,the study subjects were divided into failure group(24 cases)and success group(190 cases).Univariate and Logistic regression of EPs and patients were analyzed,and the nomogram prediction model was established and verified.Results Age>35 years,history of HRT treatment,hypertension,endometritis,and postoperative pregnancy were all high-risk factors for natural pregnancy failure in EPs.The nomogram model fitted well,and showed strong predictive ability after model validation.Conclusion Analysis of the incidence characteristics of EPs in childbearing age women and the risk factors of natural pregnancy failure after hysteroscopy can help to improve the prognosis level of patients.
