年龄大于60岁难治性反流性食管炎患者经口胃镜分级及与肥胖的关系

Relationship between Oral Gastroscopy Grading and Obesity in Refractory Reflux Esophagitis Patients over 60 Years Old

目的:探讨年龄>60岁难治性反流性食管炎(RE)患者经口胃镜分级与肥胖的关系.方法:选择2019年6月至2022年9月本院收治的103例年龄>60岁RE患者为研究对象,根据不同的胃镜分级将患者分为A~B组(n=62,胃镜分级A级+B级)和C~D组(n=41,胃镜分级C级+D级).肥胖检测指标选用腰臀比、腰围和人体质量指数(BMI).采用多因素非条件Logistic回归分析法分析患者经口胃镜分级与肥胖的关系.结果:两组性别、腰围、高血压、糖尿病、高脂血症所占比比较,差异无统计学意义(P>0.05).C~D组的年龄(>75岁)、BMI(≥28kg/m^(2))、腰臀比(≥1.00)所占比明显高于A~B组,差异有统计学意义(P<0.05).经单因素分析结果显示,年龄、BMI、腰臀比可能为年龄>60岁难治性RE患者胃镜分级的影响因素.Logistic回归分析结果显示,年龄>75岁、BMI≥28kg/m^(2)、腰臀比≥1.00是年龄>60岁难治性RE患者胃镜分级为C~D级的独立危险因素(P<0.05).结论:年龄、肥胖与年龄>60岁难治性RE的发生密切相关,是难治性RE胃镜分级的影响因素.

Objective:To analyze the relationship between the oral gastroscopy grading and obesity in patients with refractory reflux esophagitis(RE)over 60 years old.Methods:The clinical data of 103 patients over 60 years old with refractory RE treated in our hospital from June 2019 to September 2022 were retrospectively analyzed.According to different endoscopy grades,the patients were divided into groups A~B(n=62,gastroscopy grade A+B)and C~D group(n=41,gastroscope grade C+D).The obesity was measured by waist hip ratio,waistline and body mass index(BMI).Logistic regression analysis was used to analyze the relationship between the grade of oral gastroscopy and obesity.Results:There were no significant differences in the proportions of gender,waistline,hypertension,diabetes,and hyperlipidemia between the two groups(P>0.05).The proportions of age(over 75 years old),BMI(≥28kg/m^(2)),and waist hip ratio(≥1.00)in group C~D were significantly higher than those in groups A~B(P<0.05).The results of univariate analysis showed that age,BMI,and waist hip ratio may be the influencing factors of gastroscopy grading in patients over 60 years old with refractory RE.The results of Logistic regression analysis showed that age over 75 years old,BMI≥28 kg/m^(2),and waist hip ratio≥1.00 were independent risk factor for refractory RE patients over 60 years old with gastroscopy grade C~D(P<0.05).Conclusion:As the influencing factors of gastroscopic grading of refractory RE,age and obesity are closely related to the occurrence of refractory RE.
