后臂屈伸锻炼联合Mulligan动态关节松动术治疗黏连性肩周炎患者的疗效分析

Effects of Posterior Arm Flexion and Extension Exercises Combined with Mulligan Dynamic Arthrodesis in the Treatment of Patients with Adhesive Scapulohumeral Periarthritis

目的:探讨后臂屈伸锻炼联合Mulligan动态关节松动术治疗黏连性肩周炎患者的疗效.方法:选取2019年6月至2022年8月在本院就诊的86例黏连性肩周炎患者为研究对象,按随机数字表法将其分为联合组(n=43)和常规组(n=43).联合组接受后臂屈伸锻炼联合Mulligan动态关节松动术治疗,常规组接受后臂屈伸锻炼治疗.比较两组总有效率、治疗前及治疗1个月后视觉模拟评分法(VAS)及肩关节疼痛与功能障碍指数量表(SPADI)评分、患肩关节活动度.结果:治疗1个月后联合组总有效率97.67%(42/43)较常规组81.40%(35/43)高,VAS、SPADI评分较常规组低,患肩关节前屈上举、内收、后伸、旋转活动度均较常规组高,差异均有统计学意义(P<0.05).结论:后臂屈伸锻炼与Mulligan动态关节松动术联合治疗黏连性肩周炎患者可改善肩关节活动度、提升肩关节功能,以及减轻疼痛感.

Objective:To investigate effects of posterior arm flexion and extension exercises combined with Mulligan dynamic arthrodesis in the treatment of patients with adhesive scapulohumeral periarthritis.Methods:Eighty-six patients with adhesive scapulohumeral periarthritis in our hospital from June 2019 to August 2022 were selected.They were divided into combined group(n=43)and conventional group(n=43)according to the random number table method.The combined group was treated with posterior arm flexion and extension exercise combined with Mulligan dynamic arthrodesis,and the conventional group was treated with posterior arm flexion and extension exercise.The total effective rate,visual analog scoring(VAS),shoulder pain and dysfunction index scale(SPADI)scores before and after the treatment,and mobility of the affected shoulder joint were compared between the two groups.Results:After 1 month of treatment,the total effective rate of the combined group was 97.67%(42/43),which was higher than that of the conventional group 81.40%(35/43),and the VAS and SPADI scores were lower than those of the conventional group,and the forward flexion supination,internal extension,posterior extension,and rotation mobility of the affected shoulder were higher than those of the conventional group,the differences were statistically significant(P<0.05).Conclusion:The combination of posterior arm flexion and extension exercise and Mulligan's dynamic arthrodesis for patients with adhesive scapulohumeral periarthritis can further improve the efficacy,improve shoulder mobility,enhance shoulder function and reduce pain.
