普外科麻醉手术室应用"医护一体化"分层级责任制管理的效果分析

Effects of"Medical Care Integration"Hierarchical Responsibility System Management in Anesthesia Operating Room of General Surgery Department

目的:分析"医护一体化"分层级责任制管理对普外科麻醉手术室护理管理质量的影响.方法:回顾性分析本院普外科麻醉手术室实施"医护一体化"分层级责任制管理前(2019年10月至2020年5月)、实施后(2020年6月至2020年12月)护理管理相关资料.比较实施"医护一体化"分层级责任制管理前后麻醉准备时间、苏醒时间、拔管时间、麻醉相关并发症及护理差错事件发生率.结果:实施医护一体化分层责任制管理后,全身麻醉、椎管内麻醉、局部麻醉准备时间、握拳时间、睁眼时间及拔管时间均短于实施前,差异有统计学意义(P<0.05);麻醉相关并发症、护理差错事件总发生率均低于实施前,差异有统计学意义(P<0.05).实施前、后患者自主呼吸时间比较,差异无统计学意义(P>0.05).结论:外科麻醉手术室应用医护一体化分层责任制,可有效缩短麻醉准备时间及拔管时间,促进麻醉苏醒,降低麻醉相关并发症及护理差错事件的发生.

Objective:To analyze the effects of"medical care integration"hierarchical responsibility system management on nursing management quality in anesthesia operating room in department of general surgery.Methods:A retrospective analysis was performed on nursing management-related data before the implementation(from October 2019 to May 2020)and after the implementation(from June 2020 to December 2020)of"medical care integration"hierarchical responsibility system management in anesthesia operating room of department of general surgery in our hospital.The anesthesia preparation time,wake-up time,extubation time,anesthesia-related complications,and nursing error incidents were compared before and after the implementation of"medical care integration"hierarchical responsibility system management.Results:After the implementation of medical care integration hierarchical responsibility system management,the preparation times for general anesthesia,intraspinal anesthesia and local anesthesia,fist-making time,eye-opening time and extubation time were all shorter than those before the implementation,and the differences were statistically significant(P<0.05).The total incidence rates of anesthesia-related complications and nursing error events were lower than those before the implementation,the differences were statistically significant(P<0.05).There was no significant difference in the spontaneous breathing time before and after the implementation(P>0.05).Conclusion:"Medical care integration"hierarchical responsibility system in anesthesia operating room of department of general surgery can effectively shorten the anesthesia preparation time and extubation time,promote anesthesia recovery,reduce the occurrence of anesthesia-related complications and nursing error events.
