互联网远程血糖管理模式对孕期糖尿病患者妊娠结局的影响

Influence of Internet-based remote blood glucose management mode on blood glucose control and pregnancy outcome in patients with gestational diabetes

目的:探讨互联网远程血糖管理模式对孕期糖尿病患者血糖控制及妊娠结局的影响。方法:选取就诊的孕期糖尿病患者100例为研究对象,随机分为观察组和对照组各50例。对照组接受常规门诊血糖管理,观察组采用互联网远程血糖管理。比较两组患者血糖控制水平、依从性、满意度及妊娠结局的差异,分析影响妊娠结局的因素。结果:管理后,观察组空腹血糖(FPG)、餐后2 h血糖(2 hPG)水平明显低于对照组,两者达标率明显优于对照组,差异均有统计学意义(P<0.05);两组管理后糖化血红蛋白(HbA1c)水平比较差异无统计学意义(P>0.05);观察组的依从性及满意度均显著高于对照组,差异有统计学意义(P<0.05);观察组的剖宫产、产后出血、巨大儿及胎儿呼吸窘迫的发生率显著低于对照组(P<0.05);Logistic回归分析显示,年龄、FPG、2 hPG及依从性与不良妊娠结局存在显著相关性(P<0.05),其中年龄、FPG、2 hPG是危险因素,依从性为保护因素。结论:互联网远程血糖管理模式可有效控制孕期糖尿病患者的血糖水平,减少不良妊娠结局,提高依从性和满意度。

Objective To explore the influence of Internet remote blood glucose management mode on blood glucose control and pregnancy outcome of gestational diabetic patients.Method A total of 100 gestational diabetic patients who came to our hospital were enrolled as the study subjects.The random number table was adopted to divide the patients into observation group and control group,each with 50 cases.The control group received routine outpatient blood glucose management,while the observation group used Internet remote blood glucose management.The differences of blood glucose control level,compliance,satisfaction and pregnancy outcome between the two groups were compared,and the factors affecting pregnancy outcome were analyzed.Results After management,the levels of FPG and 2hPG in the observation group were significantly lower than those in the control group,and their compliance rates were significantly higher than those in the control group(P<0.05).There was no significant difference in HbA1c level between the two groups(P>0.05).The compliance and satisfaction of the observation group were significantly higher than those of the control group(P<0.05).The incidence of cesarean section,postpartum hemorrhage,macrosomia and fetal respiratory distress in the observation group was significantly lower than that in the control group(P<0.05).Logistic regression model showed that age,FPG,2hPG and compliance were significantly correlated with adverse pregnancy outcomes(P<0.05),among which age,FPG and 2hPG were risk factors and compliance was protective factors.Conclusion Internet blood glucose management model can effectively control the blood glucose level of gestational diabetes patients,reduce adverse pregnancy outcomes,improve compliance and satisfaction.It is worthy of clinical application.
