左氧氟沙星对肺癌化疗后肺部感染患者血气分析指标、肺功能和细菌清除率的影响 被引量:1

摘要 目的探讨左氧氟沙星对肺癌化疗后肺部感染患者血气分析指标、肺功能和细菌清除率的影响。方法依据治疗方法的不同将120例肺癌合并肺部感染患者分为对照组和观察组,每组60例,对照组患者给予头孢哌酮舒巴坦钠治疗,观察组患者在对照组基础上给予左氧氟沙星治疗。比较两组患者的细菌清除率、血气分析指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、pH值]、肺功能指标[第一秒用力呼气容积(FEV_(1))、用力肺活量(FVC),计算FEV_(1)/FVC]、不良反应发生情况。结果观察组患者的细菌清除率为96.67%,高于对照组患者的85.00%,差异有统计学意义(P<0.05)。治疗后,两组患者PaO_(2)、PaCO_(2)、pH值均高于本组治疗前,且观察组患者PaO_(2)、PaCO_(2)、pH值均高于对照组,差异均有统计学意义(P<0.05)。治疗后,两组患者FVC、FEV_(1)、FEV_(1)/FVC均高于本组治疗前,且观察组患者FVC、FEV_(1)、FEV_(1)/FVC均高于对照组,差异均有统计学意义(P<0.05)。两组患者的不良反应均在未治疗的情况下自行缓解,且均无肝肾功能方面的异常变化。结论化疗后肺部感染的肺癌患者给予左氧氟沙星治疗可快速控制机体的炎症反应,有助于肺功能恢复,可改善患者的血气分析指标,安全性较高,细菌清除率较高。 Objective To investigate the effects of levofloxacin on blood gas analysis index,lung function,and bacterial clearance in patients with lung infection after chemotherapy for lung cancer.Method A total of 120 patients with lung cancer complicated with lung infection were divided into control group and observation group according to different treatment methods,with 60 cases in each group.Patients in the control group were treated with cefoperazone sulbactam sodium,and the patients in the observation group were treated with levofloxacin on the basis of the control group.The bacterial clearance,blood gas analysis indexes[arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),pH value],pulmonary function indexes[forced expiratory volume in one second(FEV_(1)),forced vital capacity(FVC),FEV_(1)/FVC],and the occurrence of adverse events were compared between the two groups.Result The bacterial clearance in the observation group was 96.67%,which was higher than 85.00% in the control group,and the difference was statistically significant(P<0.05).After treatment,the PaO_(2),PaCO_(2),and pH value of both groups were higher than those before the treatment,and the PaO_(2),PaCO_(2),and pH value of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).After the treatment,FVC,FEV_(1),FEV_(1)/FVC in both groups were higher than those before the treatment,and the above indexes in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Adverse events in both groups resolved spontaneously without treatment and there were no abnormal changes in liver and kidney function.Conclusion Levofloxacin treatment for lung cancer patients with lung infection after chemotherapy can quickly control the inflammatory reaction of the body,help the recovery of lung function,improve the blood gas analysis indicators of patients,with high safety and high bacterial clearance rate.
