维持性血液透析患者依从性的影响因素分析

Analysis of influencing factors on compliance of maintenance hemodialysis patients

目的 分析维持性血液透析患者依从性的影响因素,为提高患者依从性制订措施提供参考。方法 回顾性分析2022年3~12月我院维持性血液透析患者383例的临床资料,采用单因素分析和Logistic回归分析维持性血液透析患者依从性的危险因素。结果 本研究383例维持性血液透析患者中,未按时回院行血液透析治疗53例(13.8%)。单因素分析结果显示,不同年龄、血液透析年限、费用支付方式、家庭人均月收入、是否家庭支持及并发症方面在维持性血液透析患者依从性比较,差异具有统计学意义(P<0.05)。Logistic回归分析显示,年龄[OR=2.198,95%CI(1.050,4.597]、血液透析年限[OR=2.164,95%CI(1.076,4.352)]、费用支付方式[OR=17.559,95%CI(5.634,54.727)]是影响维持性血液透析患者依从性的独立危险因素(P<0.05);家庭人均月收入[OR=0.260,95%CI(0.117,0.578)]、家庭支持[OR=0.421,95%CI(0.194,0.914)]是维持性血液透析患者依从性的保护因素。结论 加强维持性血液透析患者的风险评估与宣教,了解患者的情况并及早给予个性化干预,提高患者的依从性,保证血液透析质量。

Objective To analyze the factors affecting the compliance of maintenance hemodialysis patients,and to provide reference for the formulation of measures to improve the compliance of patients.Methods The clinical data of 383 patients with maintenance hemodialysis in our hospital from March 2022 to December 2022 were retrospectively analyzed.Univariate analysis and Logistic regression were used to analyze the risk factors of compliance in maintenance hemodialysis patients.Results This group of 383 maintenance hemodialysis patients,53 cases(13.8%)did not return to hospital on time for hemodialysis treatment.The results of univariate analysis showed that the compliance of maintenance hemodialysis patients with different ages,years of hemodialysis,payment methods,per capita monthly family income,whether family support and complications were compared.The difference was statistically significant(P<0.05).Logistic regression analysis showed that:Age[OR=2.198,95%CI(1.050,4.597)],hemodialysis years[OR=2.164,95%CI(1.076,4.352)],payment method[OR=17.559,95%CI(5.634,54.727)]was an independent risk factor affecting the compliance of maintenance hemodialysis patients(P<0.05);Family per capita monthly income[OR=0.260,95%CI(0.117,0.578)]and family support[OR=0.421,95%CI(0.194,0.914)]were protective factors for compliance in maintenance hemodialysis patients.Conclusion Strengthen the risk assessment and education of patients with maintenance hemodialysis,take the initiative to understand the situation of patients,and conduct personalized intervention as early as possible to improve the compliance of patients.
