ZHANG Dongyue’s Medication Regularity in Treating Slow Transit Constipation

目的:通过数据挖掘方法探讨张东岳治疗慢传输型便秘的处方思路及用药规律,为临床治疗慢传输型便秘提供参考。方法:收集整理2016年1月至2021年12月张东岳诊治慢传输型便秘的首诊中药处方,运用中医传承计算平台(V3.0)软件进行药物组方规律分析。结果:共纳入处方556首,涉及中药91味,药物总频次8670次。用药以补虚药为主,其次为化痰止咳平喘药和理气药;四气频数分布以温、平、寒为主;五味分布中甘、苦、辛味居多;归经以脾、肺、大肠、肝经为主;关联规则分析得到63个常用药物组合;k-means聚类分析得到核心药物组合5组。结论:张东岳治疗慢传输型便秘用药以补虚为主,寒温并用;主张多脏调變,重视健脾益肺、润燥助运;善用仁类,柔润滑利;因人制宜,临床效果显著。

Objective:To study ZHANG Dongyue’s prescription ideas and medication rules for the treatment of slow transit constipation through data mining method,and provide references for the clinical treatment of slow transit constipation.Methods:The first Chinese medicinal prescriptions of ZHANG Dongyue for the diagnosis and treatment of slow transit constipation from January 2016 to December 2021 were collected and sorted out,and the medication regularity was analyzed by using Traditional Chinese Medicine Inheritance Computing Platform(V3.0)software.Results:A total of 556 prescriptions were included,involving 91 Chinese medicinals,with the total frequency of 8670 times.The most frequently used medicinals were tonic medicinals,followed by medicinals for resolving phlegm,relieving cough and asthma and medicinals for regulating qi.The frequency distribution of the four properties was mainly warm,neutral and cold,and the distribution of the five flavors was mainly sweet,bitter and acrid.The channel tropism is mainly the spleen,lung,large intestine and liver meridians.Total 63 commonly used medicinal combinations were obtained by association rule analysis.Five groups of core medicinal combinations were obtained by K-means clustering analysis.Conclusions:ZHANG Dongyue’s medication for treating slow transit constipation mainly focuses on tonifying deficiency,using a combination of cold and warm.He advocates multiple organs regulating,emphasizes invigorating spleen and replenishing lung,moistening dryness and assisting movement.He makes good use of moistening and lubricating kernel medicinals.He treats slow transit constipation based on patient’s individual constitution,with significant clinical effects.
