12例眼眶结膜下脂肪疝临床病理分析

Orbital subconjunctival fat hernia:a clinicopathological analysis of twelve cases

目的总结分析眼眶结膜下脂肪疝的临床病理特征。方法回顾性分析本院诊断的12例眼眶结膜下脂肪疝的临床病理资料,总结眼眶结膜下脂肪疝的临床表现及病理特征。结果12例患者中,男7例,女5例,中位年龄63岁,纤维间质内可见花环状细胞及Lochkern细胞,花环状细胞CD34细胞质阳性表达。结论眼眶结膜下脂肪疝与脂肪瘤是两种不同的病变,应结合临床表现及影像学进行鉴别;眼眶结膜下脂肪疝无特异性病理诊断特征,可见花环状多核巨细胞、纤维隔膜及Lochkern细胞,这些细胞可能是一种反应性改变,应注意与多形性脂肪瘤和非典型脂肪性肿瘤相鉴别。

Objective To summarize and analyze the clinicopathological features of orbital subconjunctival fat hernia.Methods The clinicopathological data of 12 cases of orbital subconjunctival fat hernia diagnosed in our hospital were retrospectively analyzed,and the clinical manifestations and pathological characteristics of orbital subconjunctival fat hernia were summarized.Results Among the 12 patients,there were 7 males and 5 females,with a median age of 63 years.The rosette cells and Lohkern cells were found in the fibrous stroma,and the rosette cells positively expressed CD34 in the cytoplasm.Conclusion Orbital subconjunctival fat hernia and lipoma are two different lesions,which should be differentiated based on clinical manifestations and imaging studies;there are no specific diagnostic features of orbital subconjunctival fat hernia,but rosette multinucleated giant cells,fibrous septa,and Lochkern cells can be seen,which may be a reactive change and should be differentiated from pleomorphic lipomas and atypical fatty tumors.
