利拉鲁肽对老年2型糖尿病患者血糖波动及骨代谢指标的影响

Effects of Liraglutide on Blood Glucose Fluctuation and Bone Metabolism Indexes in Elderly Patients with Type 2 Diabetes Mellitus

目的研究利拉鲁肽对于老年2型糖尿病(diabetes mellitus type2,T2DM)患者血糖波动以及骨代谢指标的影响。方法选取2022年1—12月晋江市中医院内分泌科收治的老年T2DM患者80例,采用平行对照,以随机数表法分为治疗组和对照组,每组40例,治疗组给予皮下注射利拉鲁肽联合口服二甲双胍治疗,对照组仅予以口服二甲双胍治疗,比较治疗前后血糖波动和骨代谢指标变化,不良反应发生情况。结果治疗前,两组患者的空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)、一日内平均血糖波动幅度(MAGE)与骨钙素N端中分子片段(N-MID)、Ⅰ型胶原羧基前端肽(t-P1NP)、β-胶原降解产物测定(β-CTX)、维生素D3水平比较,差异无统计学意义(P>0.05);治疗后,两组FPG、2 hPG、HbA1c、MAGE、β-CTX均降低,N-MID、t-P1NP、维生素D3均升高;且治疗组FPG、2 hPG、HbA1c、MAGE、β-CTX均明显低于对照组,N-MID、tP1NP、维生素D3明显高于对照组,差异有统计学意义(P<0.05)。用药期间,两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论利拉鲁肽可有效控制老年T2DM患者血糖水平及血糖波动,有效改善骨代谢,且安全性较好。

Objective To study the effect of liraglutide on blood glucose fluctuation and bone metabolism indexes in elderly patients with(type 2 diabetes mellitus,T2DM).Methods A total of 80 elderly patients with T2DM who were admitted to the Endocrinology Department of Jinjiang Hospital of Traditional Chinese Medicine from January to December 2022 were selected.Using parallel control,they were divided into treatment group and control group by random number table method,40 cases in each group.The treatment group was given subcutaneous injection of liraglutide combined with oral metformin,while the control group was given oral metformin only.The changes in blood glucose fluctuations and bone metabolism indicators were compared before and after treatment,and adverse reactions were observed.Results Before treatment,fasting blood glucose(FPG),2-hour postprandial blood glucose(2 hPG),glycosylated hemoglobin(HbA1c),and average range of blood glucose fluctuations within a day(MAGE),osteocalcin Nterminal molecular fragment(N-MID),typeⅠcollagen carboxyl front peptide(t-P1NP),β-collagen degradation product(β-CTX),and vitamin D3 levels had no statistically significant differences(P>0.05).After treatment,FPG,2 hPG,HbA1c,and MAGE,β-CTX in both groups decreased,N-MID,t-P1NP,vitamin D3 increased.FPG,2 hPG,HbA1c,MAGE and β-CTX in the treatment group were significantly lower than those in the control group,N-MID,t-P1NP,and vitamin D3 were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).During the medication period,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Liraglutide can effectively control blood glucose level and blood glucose fluctuation in elderly patients with T2DM,effectively improve bone metabolism,and has good safety.
