情绪ABC理论认知教育在肺癌患者及家属中的应用

摘要 目的:探讨情绪ABC理论认知教育对肺癌患者及其家属情绪及患者生活质量的影响。方法:采用便利抽样法,选取2019年1月—2022年1月郑州大学第一附属医院收治的111例肺癌患者及其111名主要照顾者为研究对象。将2019年1月—2020年6月纳入的55例患者及55名主要照顾者设为对照组,将2020年7月—2022年1月纳入的56例患者及56名主要照顾者设为试验组。对照组主要照顾者采用常规认知教育方式,试验组主要照顾者在对照组基础上采用基于情绪ABC理论下的认知教育。采用正性负性情绪量表(PANAS)、肺癌患者生活质量测定量表(FACT-L)对患者及其主要照顾者进行调查,比较干预前后两组主要照顾者及患者的负性情绪以及两组患者生活质量。结果:干预前两组主要照顾者及患者的PANAS评分比较差异均无统计学意义(P>0.05);干预后两组主要照顾者及患者的PANAS评分均降低,且试验组评分低于对照组,差异均有统计学意义(P<0.05)。干预前两组患者FACT-L中生理状况、社会/家庭状况、与医生的关系、情感状况、功能状况、附加的关注评分及总分比较差异均无统计学意义(P>0.05);干预后试验组患者生理状况、社会/家庭状况、与医生的关系、情感状况、功能状况、附加的关注评分及总分均高于对照组,差异均有统计学意义(P<0.05)。结论:基于情绪ABC理论下肺癌患者主要照顾者认知教育可减轻主要照顾者及患者的负性情绪,提高患者生活质量。 Objective To explore the effect of emotional ABC theory-based cognitive education on the emotions of lung cancer patients and their families and quality of life of patients.Methods From January 2019 to January 2022,111 lung cancer patients and 111 main caregivers admitted to the First Affiliated Hospital of Zhengzhou University were selected as the study subject by convenience sampling.55 patients and 55 main caregivers enrolled from January 2019 to June 2020 were assigned to the control group,and 56 patients and 56 main caregivers enrolled from July 2020 to January 2022 were assigned to the experimental group.The main caregivers in the control group adopted conventional cognitive education,while the main caregivers in the experimental group adopted cognitive education based on emotional ABC theory on the basis of the control group.A survey was conducted on patients and their main caregivers using the Positive and Negative Affect Scale(PANAS)and the Functional Assessment of Cancer Therapy-Lung(FACT-L)to compare the negative emotions of the two groups of main caregivers and patients,as well as quality of life of patients before and after intervention.Results There was no statistically significant difference in PANAS scores between the two groups of main caregivers and patients before intervention(P>0.05).After intervention,the PANAS scores of the main caregivers and patients in both groups decreased,and the scores in the experimental group were lower than those in the control group,with statistical differences(P<0.05).There was no statistically significant difference in scores of physical well being,social/family well being,relationship with doctor,emotional well being,functional well being,additional attention,and total score in FACT-L between the two groups of patients before intervention(P>0.05).After intervention,the scores of physical well being,social/family well being,relationship with doctor,emotional well being,functional well being,additional attention,and total score in FACT-L of the experimental group patients were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusions Emotional ABC theory-based cognitive education for lung cancer patients and their main caregivers can alleviate the negative emotions of the main caregivers and patients,and improve patients'quality of life.
