多层螺旋CT诊断急性阑尾炎的价值分析

Analysis of the value of multi-slice spiral CT in the diagnosis of acute appendicitis

目的探讨多层螺旋CT诊断急性阑尾炎的价值。方法选择2017年1月-2021年12月期间常州市金坛区第一人民医院收治的疑似急性阑尾炎患者95例,均接受日本东芝公司Aquilion 64排螺旋CT扫描,评价多层螺旋CT诊断的敏感度、特异度及准确率,并比较不同病理类型阑尾炎的检出率及CT征象。结果手术和病理检出急性阑尾炎80例,检出率为84.21%,对急性单纯性阑尾炎、急性化脓性阑尾炎、急性坏疽性阑尾炎、阑尾周围脓肿等检出率为33.68%,23.16%,16.84%,10.53%,多层螺旋CT总计检出率为85.26%,不同病理阑尾炎检出率为35.79%,22.11%,16.84%,10.53%,与手术病理检出率比较差异无统计学意义(P>0.05)。多层螺旋CT诊断急性阑尾炎的准确率为96.84%,敏感度98.75%,特异度86.67%。结论急性阑尾炎患者行多层螺旋CT诊断具有较高的敏感度、特异度及准确率,而且可依据征象对不同病理类型阑尾炎行鉴别诊断,为临床诊治提供可靠依据,值得推广。

Objective To investigate the accuracy and application effect of multi-slice spiral CT in the diagnosis of acute appendicitis.Methods A total of 95 patients with suspected acute appendicitis admitted to the hospital from January 2017 to December 2021 were selected as the research data,and 80 patients with acute appendicitis were diagnosed by surgery and pathology.Diagnosis was carried out,the sensitivity,specificity and accuracy of multi-slice spiral CT were evaluated,and the detection rate and CT signs of different pathological types of appendicitis were compared.Results 80 cases of acute appendicitis were detected by surgery and pathology,and the detection rate was 84.21%.The detection rates of acute simple appendicitis,acute suppurative appendicitis,acute gangrenous appendicitis,and periappendiceal abscess were 33.68%,23.16%,and 16.84%,10.53%,the total detection rate of multi-slice spiral CT was 85.26%,and the detection rates of different pathological appendicitis were 35.79%,22.11%,16.84%,and 10.53%,with no statistical significance(P>0.05).The accuracy of muli-slice spiral CT in the diagnosis of acute appendicitis was 96.84%,the sensitivity was 98.75%,and the specificity was 86.67%.Conclusion Multi-slice spiral CT has high sensitivity,specificity and accuracy in the diagnosis of acute appendicitis,and it can be used for differential diagnosis of different pathological types of appendicitis according to the signs,which provides a reliable basis for clinical diagnosis and treatment,and is worthy of promotion.
