“从阴引阳”针法治疗颈肩肌筋膜疼痛综合征经验总结

The experience in treating neck shoulder myofascial pain syndrome by the method of acupuncture at Yin meridian acupoint to treat Yang meridian disease

肌筋膜疼痛综合征(Myofascial Pain Syndrome,MPS)是一种会反复发作的慢性疼痛疾病,其主要症状为肌筋膜组织局部疼痛、固定压痛和肌肉紧张而僵硬等,其中以枕、颈、肩、下背以及股部症状较为多见,在颈肩部发病的为颈肩MPS。此病多因患者肌肉力学关系紊乱、肌群过度紧张而导致。治疗此类病证的传统针刺方法的疗效不甚理想。福建中医药大学附属龙岩市中医院袁世伟副主任以《黄帝内经》针刺理论为基础,经过反复临床验证,结合现代社会群体均存在力线失衡状态,总结出了“从阴引阳”针法治疗MPS。此疗法有别于传统治疗痹证的“以痛为腧、局部取穴”原则,对因力学关系紊乱而变紧张的肌群进行针刺,使其逐渐恢复正常功能关系,恢复肌肉与肌肉之间、肌群与肌群之间原有的运动生物力学上的平衡,从本质上治愈MPS,已取得较好的临床效果,为针刺治疗MPS提供了新的思路,值得广泛运用。文章结合案例介绍“从阴引阳”针法治疗MPS经验,以期为临床拓展思路。

Myofascial pain syndrome(MPS)is a recurrent chronic pain disorder.The main symptoms of the patients are local pain of myofascial tissue,fixed tenderness,and muscle tension and stiffness,among which the symtoms of the head back,neck,shoulder,lower back and thigh are more common,and this disease in the neck and shoulder is known as neck and shoulder MPS.Neck and shoulder MPS is mostly caused by muscle mechanical relation disorder and excessive muscle tension.Traditional acupuncture method on this kind of disease shows no ideal efficacy.YUAN Shiwei,a deputy director of Longyan Hospital of Traditional Chinese Medicine affiliated to Fujian University of Traditional Chinese Medicine,based on the acupuncture theory in Huangdi Neijing(《黄帝内经》),and combined with repeated clinical verification,summarized the method of acupuncture at Yin(阴)meridian acupoint to treat Yang(阳)meridian disease to treat MPS.This therapy is different from the traditional treatment of paralysis,which uses pain as the acupoint and takes the acupoint locally,and acupunctures are performed on the muscle groups that have become tense due to mechanical relationship disorders,so that they can gradually return to normal functional relationships.This therapy can restore the original mechanical balance between muscles and between muscle groups,essentially cure MPS,and has achieved good clinical results.It provides a new idea for acupuncture treatment of MPS,and is worthy of extensive application.In this article,the experience of treating MPS by the method of acupuncture at Yin meridian acupoint to treat Yang meridian disease is introduced,hoping to expand the treatment thinking for clinical practice.
