三晋王氏妇科治疗痛经病临证经验

The clinical experience of WANG’s gynaecological school in Sanjin area in treating dysmenorrhea

痛经病是妇科常见病,严重影响广大女性的生活质量。三晋王氏妇科流派为国家级非物质文化遗产,始于北宋,历经千年。王金权教授为其第28代传人,王氏中医妇科流派学术带头人,在妇科痛经病的诊治中有着丰富的临床经验和独到的学术思想。王金权教授认为痛经病在肝、脾、肾三脏,病位在胞宫,病机不外乎“不通则痛”“不荣则痛”两种。王教授将本病分为气滞血瘀、寒湿凝滞伴血瘀、湿热瘀阻、气血亏虚伴血瘀、肾虚夹瘀五个证型。导师认为治疗上应统筹三脏,肝、脾、肾三脏兼顾;根据证型辨证用药,并顺应周期变化,灵活调周用药。针对痛经病,王教授在治疗上辨证精准,选方精妙,用药灵活,疗效显著。文章通过案例介绍王金权教授治疗痛经病的经验,以期为临床提供参考。

Dyenorrhea is a common gynaecological disease,seriously affecting the life quality of women.WANG’s gynaecological school in Sanjin area(三晋王氏妇科流派)is a national intangible cultural heritage,which began in the Northern Song Dynasty(北宋时期)and has lasted for thousands of years.Professor WANG Jinquan is the 28th generation descendant and the academic leader of WANG’s gynaecological school of TCM in Sanjin area.The professor has rich clinical experience and unique academic thoughts in the diagnosis and treatment of gynaecological dysmenorrhea.Professor WANG Jinquan believes that the occurrence of dysmenorrhea is in the liver,spleen and kidneys,and the disease is located in the uterus,and the disease mechanism is nothing more than pain if it is not smooth or it is not nourished.Professor WANG divides the disease into five types:Qi(气)and blood stasis,cold and wet coagulation with blood stasis,humid heat stasis,Qi and blood deficiency with blood stasis,and kidney deficiency and stasis.The instructor believes that the treatment should coordinate the three organs,and take into account the liver,spleen and kidney.According to the type of differentiation of medication,and adapt to the change of cycle,flexible weekly medication.For the treatment of dysmenorrhea,Professor WANG carries out accurate syndrome differentiation,exquisite prescription selection,and flexible medication,and has obtained remarkable curative effects.In this paper,Professor WANG Jinquan’s experience in the treatment of dysmenorrhea is introduced through an empirical case,so as to provide a reference for the clinical practice.
