清肺消痈汤联合纤维支气管镜肺泡灌洗吸痰治疗重症肺炎的效果

Effect of Qingfei Xiaoyong Decoction Combined with Bronchofiberscope Alveolar Lavage and Sputum Aspiration in the Treatment of Severe Pneumonia

目的分析清肺消痈汤联合纤维支气管镜肺泡灌洗吸痰治疗重症肺炎的临床效果。方法前瞻性纳入2019年11月至2021年11月武警河南省总队医院收治的重症肺炎患者共84例,分为对照组及观察组,各42例。在常规治疗基础上,对照组接受纤维支气管镜肺泡灌洗吸痰,观察组接受纤维支气管镜肺泡灌洗吸痰联合清肺消痈汤,两组均连续治疗7 d。比较两组治疗临床效果,治疗前、治疗7 d时病情严重程度,病情恢复情况,治疗期间不良情况发生情况。结果治疗7 d,观察组总有效率较对照组高(P<0.05);两组急性生理学及慢性健康状况Ⅱ(APACHEⅡ)、临床肺部感染评分(CPIS)均降低,且观察组低于对照组(P<0.05);观察组CD4^(+)、CD4^(+)/CD8^(+)高于对照组,CD8^(+)低于对照组(P<0.05);观察组机械通气时间和住院时间短于对照组(P<0.05);组间不良反应对比差异无统计学意义(P>0.05)。结论清肺消痈汤联合纤维支气管镜肺泡灌洗吸痰治疗重症肺炎的临床效果好,可降低病情严重程度,提高机体免疫功能,促进身体恢复,安全性可。

Objective To analyze the clinical effect of Qingfei Xiaoyong decoction combined with bronchofiberscope alveolar lavage and sputum aspiration in the treatment of severe pneumonia.Methods A total of 84 patients with severe pneumonia admitted to Henan Provincial Corps Hospital of Armed Police Force from November 2019 to November 2021 were prospectively included.They were divided into control group and observation group,with 42 cases in each group.On the basis of routine treatment,the control group was given bronchofiberscope alveolar lavage and sputum aspiration,and the observation group was given Qingfei Xiaoyong decoction combined with bronchofiberscope alveolar lavage.Both groups were treated continuously for 7 days.The clinical effects of the two groups of treatment,including the severity of the condition before and 7 days after treatment,the recovery of the condition,and the occurrence of adverse events during the treatment period were compared.The clinical effects of two treatment groups,the severity of the condition before and after 7 days of treatment,the recovery of the condition,adverse events during treatment were compared.Results After 7 days of treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).The acute physiology and chronic health statusⅡ(APACHEⅡ)and clinical pulmonary infection score(CPIS)of both groups decreased,and the observation group was lower than the control group(P<0.05).The CD4^(+) and CD4^(+)/CD8^(+) levels in the observation group were higher than those in the control group,while CD8+levels were lower than those in the control group(P<0.05).The mechanical ventilation time and hospitalization time in the observation group were shorter than those in the control group(P<0.05).There was no statistically difference in the comparison of adverse reactions between groups(P>0.05).Conclusion The clinical effect of Qingfei Xiaoyong decoction combined with bronchoscopy alveolar lavage and sputum suction in the treatment of severe pneumonia is good,which can reduce the severity of the condition,improve the immune function of the body,promote physical recovery,and is safe.
