某院重点监控辅助用药综合管理模式构建及应用效果

Construction and Application of Comprehensive Management Mode for Key Monitoring Auxiliary Drugs in a Hospital

目的探讨医院重点监控辅助用药综合管理模式的构建及应用效果。方法抽取医院信息系统2017年1月1日至2021年12月31日重点监控辅助用药采购数据,并进行分析。依据政策法规,结合实际制订医院重点监控辅助用药目录及监管措施,动态监控重点监控辅助用药,加强重点监控辅助用药信息化管理平台建设及合理用药宣传培训,分析成效。结果制订了《医院重点监控辅助用药管理规定》等管理制度及医院重点监控辅助用药目录,目录共包含50种药品。2017年至2021年,国家级及院级重点监控辅助用药采购金额及占比均逐年降低,分别从2017年的4082.66万元(12.63%)、7011.92万元(21.70%)降至2021年的0元(0)、528.22万元(1.32%);年采购金额排名前10的重点监控辅助用药采购金额及占比从2017年的1366.53万元(4.23%)降至2021年的421.80万元(1.06%)。年医嘱合理率总体上升,由2017年的82.41%升至2021年的95.96%。结论重点监控辅助用药综合管理模式可促进重点监控辅助用药的规范化管理,提高合理用药水平。

Objective To investigate the construction and application effect of the comprehensive management mode for key monitoring auxiliary drugs in hospital.Methods The procurement data of key monitoring auxiliary drugs in the hospital information system from January 1,2017 to December 31,2021 were extracted and analyzed.According to the policies and regulations,the catalogue of key monitoring auxiliary drugs and regulatory measures in hospital were formulated based on the actual use.The key monitoring auxiliary drugs were monitored dynamically,the construction of information-based management platform of key monitoring auxiliary drugs,the propaganda and training of rational drug use were strengthened.Then,the application effect of the comprehensive management mode was analyzed.Results The Regulations on the Management of Key Monitoring Auxiliary Drugs in Hospital,other management systems and a catalogue of key monitoring auxiliary drugs in hospital(containing 50 varieties)were formulated.From 2017 to 2021,the procurement amount and proportion of national key monitoring auxiliary drugs decreased year by year,from CNY 40.8266 million(12.63%)in 2017 decreased to CNY 0(0)in 2021,and those of hospital-level key monitoring auxiliary drugs also decreased year by year,from CNY 70.1192 million(21.70%)in 2017 to CNY 5.2822 million(1.32%)in 2021.The procurement amount and proportion of top 10 key monitoring auxiliary drugs with more annual procurement amount decreased from CNY 13.6653 million(4.23%)in 2017 to CNY 4.2180 million(1.06%)in 2021.The annual rational rate of medical advice review generally increased,from 82.41%in 2017 to 95.96%in 2021.Conclusion The comprehensive management mode for key monitoring auxiliary drugs can promote the standardized management and rational use of these drugs.
