摘要
目的分析锥形束CT(CBCT)辅助开窗减压术治疗颌骨囊肿疗效及术后成骨效果的影响。方法前瞻性选取2017年1月至2022年1月宿迁市第一人民医院收治的60例颌骨囊肿患者,按照随机数字表法分为观察组和对照组,每组各30例,其中对照组患者给予刮治术治疗,观察组患者给予CBCT辅助开窗减压术治疗。对比两组患者的手术时间、住院时间、术中出血量以及不良反应发生率,对比2组治疗前后视觉模拟评分法(VAS)评分及囊腔体积缩小程度、治疗后切口愈合情况。结果观察组患者手术时间、住院时间分别为(6.12±0.53)min、(5.40±0.82)d,均短于对照组[(54.42±1.20)min、(5.73±1.01)d],术中出血量为(4.84±0.90)mL,低于对照组[(90.31±10.50)mL],差异均有统计学意义(P<0.05)。观察组患者的不良反应发生率为6.67%,低于对照组(23.33%),差异有统计学意义(P<0.05)。治疗后,两组患者的VAS评分均较治疗前降低,且观察组VAS评分为(3.06±0.76)分,低于对照组[(4.71±1.15)分],差异均有统计学意义(P<0.05)。治疗后,观察组患者的囊腔体积缩小程度为(82.93±3.74)%,大于对照组[(41.08±2.20)%],差异有统计学意义(P<0.05)。观察组患者的治疗总有效率为96.67%,高于对照组(70.00%),观察组患者切口甲愈合比率为93.33%,高于对照组(76.67%),差异均有统计学意义(P<0.05)。治疗后3、6个月,观察组患者的新生骨质骨体积分数分别为(57.92±1.37)%、(69.88±2.28)%,均高于对照组[(32.26±2.16)%、(54.41±3.33)%],差异均有统计学意义(P<0.05)。结论颌骨囊肿患者的临床治疗中,采用CBCT辅助开窗减压术具有更好的效果,治疗有效率明显提高,术后成骨效果较好,值得临床推广应用。
Objective To analyze the effect of cone-beam CT(CBCT)-assisted fenestration decompression in the treatment of jawbone cyst and the effect of postoperative osteogenesis.Methods Sixty cases of jawbone cyst admitted to The First People's Hospital of Suqian from January 2017 to January 2022 were prospectively divide into observation group and control group according to the random number table method,with 30 cases in each group.The patients in the control group were treated with curettage,and the patients in the observation group were treated with CBCT-assisted fenestration decompression.The operation time,hospital stay and bleeding volume of the two groups were compared,as well as the incidence of adverse reactions in the two groups,the visual analogue scale(VAS)score before and after treatment,the reduction of the capsule volume and the wound healing of the two groups were compared.Results The operation time and hospitalization time of the observation group were(6.12±0.53)minutes and(5.40±0.82)days,respectively,which were shorter than those of the control group[(54.42±1.20)minutes and(5.73±1.01)days],the intraoperative bleeding volume of the observation group was(4.84±0.90)mL,which was lower than that of the control group[(90.31±10.50)mL],the differences were statistically significant(P<0.05).The incidence of adverse reactions in patients in the observation group was 6.67%,which was lower than that in the control group(23.33%),the difference was statistically significant(P<0.05).After treatment,the VAS scores of both groups of patients decreased compared to those before treatment,and the VAS score of the observation group was(3.06±0.76)points,which was lower than the control group[(4.71±1.15)points],the difference was statistically significant(P<0.05).And the volume of the capsule in the observation group was(82.93±3.74)%,which was larger than that in the control group[(41.08±2.20)%],the differences were statistically significant(P<0.05).The response rate of patients in the observation group was 96.67%,which was higher than that in the control group(70.00%),the rate of grade A wound healing in the observation group was 93.33%,which was higher than that in the control group(76.67%),the differences were statistically significant(P<0.05).After 3 and 6 months of treatment,the volume fractions of newly formed bone in the observation group were(57.92±1.37)%and(69.88±2.28)%,respectively,which were higher than those in the control group[(32.26±2.16)%and(54.41±3.33)%],the differences were statistically significant(P<0.05).Conclusion In the clinical treatment of patients with jawbone cyst,CBCT-assisted fenestration decompression has a better effect,the effective rate of treatment is significantly improved,and the effect of osteogenesis after operation is good,which is worthy of clinical application.
