NBNA评分与aEEG联合应用对早产儿脑白质发育的评估及相关性研究 被引量:1

摘要 目的研究神经行为测定(NBNA)评分与振幅整合脑电图(aEEG)联合应用对早产儿脑白质发育的评估价值及相关性。方法 选取2019年1月-2022年1月眉山市人民医院收治的108例早产儿为研究对象,根据NBNA评分结果将受试者分为脑白质发育正常组(NBNA评分≥37分,52例)和异常组(NBNA评分<37分,56例)。对早产儿进行aEEG评分,比较两组间NBNA、aEEG评分及其他可能影响早产儿脑白质发育异常相关因素。采用Logistic回归分析法明确影响早产儿脑白质发育异常的危险因素。绘制受试者工作特征(ROC)曲线评价NBNA、aEEG评分对早产儿脑白质发育异常的评估价值,并采用Pearson相关性分析NBNA评分与aEEG评分的相关性。结果 异常组NBNA、aEEG评分均低于正常组(P<0.05)。Logistic回归分析显示NBNA、aEEG评分均是影响早产儿脑白质发育异常的危险因素(P<0.05)。ROC曲线分析显示,NBNA联合aEEG评分评估早产儿脑白质发育异常的灵敏度、准确度、AUC分别为92.31%、86.47%、0.822均高于NBNA评分、aEEG评分单独预测。Pearson相关性分析显示,NBNA评分与aEEG评分呈正相关(r=0.516,P<0.05)。结论 早产儿aEEG评分能反映脑白质发育情况,其与NBNA评分呈正相关,两者结合对早产儿脑白质发育情况的评估价值更高。 Objective To investigate the value and correlation of neurobehavioral assessment(NBNA)score combined with amplitude-integrated electroencephalography(aEEG)in evaluating white matter development in preterm infants.Methods A total of 108 premature infants admitted to our hospital from January 2019 to January 2022 were selected as the research subjects.According to the NBNA score,the subjects were divided into a normal white matter development group(NBNA score≥37 points,52 cases)and an abnormal group(NBNA score<37 points,56 cases).The aEEG scores of the premature infants were compared,and the NBNA,aEEG scores and other factors that may affect the abnormal white matter development of the premature infants were compared between the two groups.Logistic regression analysis was used to identify the risk factors affecting white matter development in premature infants.Receiver operating characteristic(ROC)curves were drawn to evaluate the evaluation value of NBNA and aEEG scores for white matter dysplasia in premature infants,and Pearson correlation was used to analyze the correlation between NBNA score and aEEG score.Results The NBNA and aEEG scores of the abnormal group were lower than those of the normal group(P<0.05).Logistic regression analysis showed that both NBNA and aEEG scores were risk factors for cerebral white matter dysplasia in premature infants(P<0.05).ROC curve analysis showed that the sensitivity,accuracy,and AUC of NBNA combined with aEEG score in evaluating white matter dysplasia in premature infants were 92.31%,86.47%,and 0.822,respectively,which were higher than those predicted by NBNA score and aEEG score alone.Pearson correlation analysis showed that NBNA score was positively correlated with aEEG score(r=0.516,P<0.05).Conclusion The aEEG score of preterm infants can reflect the development of white matter,and it is positively correlated with the NBNA score.
