摘要
心血管毒性是各类抗肿瘤治疗潜在的短期或长期并发症。在抗肿瘤治疗导致的心脏毒性的监测中,左房结构与功能改变在心功能维持、疾病预后中具有重要意义,越来越受到临床医师的重视。超声心动图是临床心功能评价的首选检查,常规超声心动图受声束夹角、心脏前后负荷的影响,在评估左心房功能时具有局限性。近年广泛应用的斑点追踪技术在克服角度依赖性的基础上,相比于传统超声心动图能更快识别心肌功能障碍,从而更早发现左心房功能改变。本文就斑点追踪技术对于癌症治疗相关心功能不全(Cancer therapeutics-related cardiac dysfunction,CTRCD)中左心房功能改变的评估进行综述。
Cardiovascular toxicity is either a potential short-term or a long-term complication of various anti-neoplastic therapies.In the monitoring of cardiotoxicity caused by anti-neoplastic therapy,the changes in the structure and function of left atrial are of great significance in the maintenance of cardiac function and the prognosis of disease,which have attracted more and more attention of clinicians.Echocardiography is the first choice for the evaluation of cardiac function clinically.Conventional echocardiography is affected by the angle of acoustic beam and the cardiac preload or after-load,thus having limitations in evaluating left atrial function.On the basis of overcoming angle dependence,speckle tracking imaging,which is widely used in recent years,can identify myocardial dysfunction faster than traditional echocardiography,so as to discover the changes in left atrial function earlier.This paper reviews the value of assessment of speckle tracking imaging for changes in left atrial function in cancer therapeutics-related cardiac dysfunction(CTRCD).
