自体脂肪移植联合CO_(2)点阵激光治疗中重度痤疮后凹陷性瘢痕的疗效观察

Observation on curative effect of autologous fat transplantation combined with CO_(2)fractional laser therapy in the treatment of depressed scars after moderate to severe acne

目的探索中重度痤疮后凹陷性瘢痕的治疗方案。方法纳入中重度痤疮后凹陷性瘢痕患者40例。根据治疗方式不同,分为联合组19例及对照组21例。联合组采用自体脂肪移植联合CO_(2)点阵激光治疗,对照组采用单纯CO_(2)点阵激光治疗。通过痤疮瘢痕ECCA分级评分表评价治疗前后皮损的变化,并对比上述两种方法的治疗效果和相关不良反应。结果治疗前后联合组的ECCA评分差值明显大于对照组,差异有统计学意义(P<0.05);两组激光平均脱痂时间和红斑消退情况比较,差异无统计学意义(P>0.05);对照组色素沉着情况较联合组更显著(P<0.05);联合组患者较对照组患者满意度更好(P<0.05)。结论自体脂肪移植联合CO_(2)点阵激光治疗中重度痤疮后凹陷性瘢痕疗效确切,可达到凹陷填充和改善皮肤质地的双重效果,且效果优于单纯CO_(2)点阵激光治疗。

Objective To explore the treatment options for depressed scar after moderate to severe acne.Methods Forty patients with moderate to severe acne depressed scar were included.According to different treatment options,the patients were divided into a combined group(n=19)and a control group(n=21).The combined group was treated with autologous fat transplantation combined with CO_(2)fractional laser therapy.The control group was treated with CO_(2)fractional laser therapy.The ECCA grading of skin lesions before and after treatment was evaluated.The treatment effect and related adverse reactions were compared between the two methods.Results The difference of ECCA score before and after treatment in the combined group was significantly greater than that in the control group(P<0.05).There was no significant difference in the mean laser scab time and erythema regression after surgery between the two groups(P>0.05).The control group had more pigmentation situation than the combined group did(P<0.05).The patients in the combined group had better satisfaction than the control group did(P<0.05).Conclusions The combination of autologous fat grafting with CO_(2)fractional laser therapy is effective in the treatment of depressed scar after moderate to severe acne.It would achieve the dual effect of depression filling and skin texture improvement.Its effect is better than that of CO_(2)fractional laser treatment alone.
