降尿酸方对尿酸性肾病血管保护及氧化应激的影响

Effects of the uric acid-lowering formula on the vascular protection and oxidative stress in patients with uric acid nephropathy

目的观察降尿酸方对尿酸性肾病患者血管保护及氧化应激反应的影响。方法随机选取2021年1月1日-2021年12月31日期间收治的80例尿酸性肾病患者,采用随机数表法分为西医组(40例,单纯西医治疗)与中西医结合组(40例,西医治疗加降尿酸方),对比2组治疗总有效率、血管内皮功能、肾功能、氧化应激功能、不良反应。结果中西医结合组整体治疗效果优于西医组,差异有统计学意义(Z=2.056,P=0.04);治疗结束1周,与西医组相比,中西医结合组患者血清丙二醛(MDA)、晚期蛋白氧化产物(AOPP)、血清内皮素-1(ET-1)、血尿酸(SUA)、血肌酐(SCr)、血尿素氮(BUN)较低,血清超氧化物歧化酶(SOD)、总抗氧化能力(TAOC)、一氧化氮(NO)、内生肌酐清除率(CCr)水平较高(P均<0.001);2组患者不良反应总发生率对比,P>0.05。结论降尿酸方治疗尿酸性肾病患者显著,利于改善血管内皮功能及肾功能,抑制氧化应激反应,且安全性理想。

Objective To observe the effects of the uric acid-lowering formula on the vascular protection and oxidative stress in patients with uric acid nephropathy.Methods eighty patients with uric acid nephropathy admitted to our hospital between January 1,2021 and December 31,2021 were randomly selected and divided into a western medicine group(40 cases with a western medicine treatment alone)and the combination group(40 cases with a western medicine treatment plus a uric acid-lowering formula).The total effective rates,vascular endothelial function,renal function,oxidative stress function and adverse reactions of the two groups were compared.Results The overall treatment effect of the combination group was better than that of the western medicine group,with a statistically significant difference(Z=2.056,P=0.04).One week after treatment,compared with the western medicine group,the patients in the combination group had lower levels of serum malondialdehyde(MDA),advanced protein oxidation products(AOPP),serum endothelin-1(eT-1),serum uric acid(SUA),serum creatinine(SCR),and blood urea nitrogen(BUN),while higher levels of serum superoxide dismutase(SOD),total antioxidant capacity(TAOC),nitric oxide(NO),and endogenous creatinine clearance rate(CCr)(all P<0.001).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The uric acid-lowering formula is significant in treating patients with uric acid nephropathy,which is beneficial for improving the vascular endothelial function and renal function,inhibiting the oxidative stress and having an ideal safety profile.
