基于等级反应模型的中文版PROMIS身体功能简表8b在癌症患者中的测量学属性评价

Evaluation of measurement properties for the Chinese version of PROMIS Physical Function Short-Form 8b based on the graded response model in cancer patients

目的以项目反应理论为指导,分析中文版患者报告结局测量信息系统(PROMIS)身体功能简表8b(PROMIS-PF-8b)在癌症患者群体中的测量学属性。方法于2020年11月-2022年8月,在复旦大学附属肿瘤医院和复旦大学附属上海市第五人民医院开展大样本的调研,收集就诊的癌症患者人口学资料和身体功能资料,拟合等级反应模型,基于项目反应理论评估量表的区分度、难度、信息量及项目功能差异等指标。结果最终发放1200份调查问卷,回收有效问卷1024份。结果显示,中文版PROMIS-PF-8b符合单维性和单调性的研究假设;基于等级反应模型的参数估计结果显示,各条目的区分度范围为[1.11,7.42],各条目难度范围为[-1.44,2.30],表明量表区分度理想,难度适中;量表整体最大信息量为34.768;量表的测量等价性结果显示,分别有2个条目在性别及年龄上存在项目功能差异(P<0.05)。结论中文版PROMIS-PF-8b具有良好的测量性能,可用于国内癌症患者身体活动水平的自我报告。

Objective To evaluate the psychometric proprieties of the Chinese version of Physical Function Short-Form 8b(PROMIS-PF-8b)of Patient-Reported Outcomes Measurement Information System(PROMIS)among patients with cancerguided by item response theory.Methods An extensive sample survey was designed to collect data in cancer patientsin Fudan University Shanghai Cancer Center and Shanghai Fifth People's Hospital of Fudan University from November 2020 to August 2022,and the demographic data and physical function data of cancer patients were collected,andthe graded response model was fitted,and the index of discrimination,difficulty,information,and differential item functioning(DIF)was evaluated based on item response theory.Results A total of 1200 questionnaires were distributed and 1024 valid questionnaires were collected.The results showed that the Chinese version of PROMIS-PF-8b met the research assumptions of unidimensionality and monotonicity.The parameter estimation results based on the graded response model showed that the range of discrimination of each item was[1.11,7.42],and the range of difficulty of each item was[-1.44,2.30],and the scale had good performance in terms of discrimination and difficulty.The overall maximum information of the scale was 34.768.In addition,the measurement equivalence of the scale was evaluated,and there were two items with DIF in gender and age respectively(P<0.05).Conclusion The Chinese version of PROMIS-PF-8b has good measurement performance and can be used for self-assessment of physical function for cancer patients in China.
