温针灸肾俞穴治疗绝经后骨质疏松症的疗效观察

Observation on curative effect of warm acupuncture at shenshu point in treatment of postmenopausal osteoporosis

目的探讨温针灸肾俞穴治疗绝经后骨质疏松症的疗效观察。方法选择2020年1月-2022年6月同济大学附属上海市第四人民医院针灸推拿科门诊收治60例绝经后骨质疏松症患者,采用简单数字表法分为治疗组和对照组,各30例。治疗组采用温针灸肾俞穴加钙尔奇碳酸钙D3片治疗,对照组仅采用钙尔奇碳酸钙D3片治疗。比较两组临床效果、视觉模拟疼痛(Visual analog pain,VAS)评分、骨密度T值、中医症状评分、性激素[促卵泡素(Follicle stimulating hormone,FSH)、促黄体素(Luteinizing hormone,LH)、雌二醇(Estradiol,E2)]、血清炎性因子[肿瘤坏死因子α(Tumor necrosis factor,TNF-α)、胰岛素样生长因子-1(Insulin like growth factor-1,IGF-1)、白细胞介素6(Interleukin-6,IL-6)]水平。结果治疗组总有效率(96.67%)高于对照组(73.33%),差异有统计学意义(P<0.05)。治疗2、4、8周后,治疗组的骨密度T值均高于对照组,VAS评分均低于对照组(P<0.001)。治疗8周后,治疗组腰背疼痛、下肢疼痛、酸软少力、纳差腹胀评分和TNF-α、IL-6水平均低于对照组(P<0.05),FSH、LH、E2、IGF-1水平均高于对照组(P<0.05)。结论温针灸肾俞穴联合钙剂较单纯钙剂治疗绝经后骨质疏松症效果更明显,具有临床推广价值。

Objective To investigate the curative effectof warmacupuncture at Shenshu pointin treatment of postmenopausal osteoporosis.Methods From January 2020 to June 2022,60 patients with postmenopausal osteoporosis were treated in the outpatient department of acupuncture and Massage of the hospital.The patients were divided into the treatment group and the control group by simple number table,with 30 cases in each group.The control group was treated with warm acupuncture plus calcic calcium carbonate D3 tablets,while the control group was treated with calcic calcium carbonate D3 tablets only.Compared the clinical effects,visual analog pain(VAS)score,bone mineral density T value,TCM symptom score,sex hormones[follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2)],and serum inflammatory factors[tumor necrosis factor(TNF-α),Insulin like growth factor-1(IGF-1)andinterleukin-6(IL-6)]levels.Results The total effective rate of the treatment group(96.67%)was higher than that of the control group(73.33%),with a statistically significant difference(P<0.05).At the 2nd,4th,and 8th weeks of the treatment,the T-value of bone density in the treatment group was higher than that in the control group,and the VAS score was lower than that in the control group(P<0.001).After 8 weeks of the treatment,the treatment group had lower back pain,lower limb pain,soreness,weakness,anorexia,abdominal distension score,and TNF-α,and IL-6 levelsin treatment group werelower than thosein control group(P<0.05),and the levels of FSH,LH,E2,and IGF-1 were higher than those in control group(P<0.05).Conclusion The combination of warm needling at Shenshu point and calcium therapy has a more significant effect on postmenopausal osteoporosis compared to simple calcium therapy,and it has clinical promotion value.
