家庭环境特征对儿童人工耳蜗植入后听力及言语发育的影响

Effects of family environment on hearing and speech development of children after cochlear implantation

目的 探讨家庭环境特征对语前聋患儿耳蜗植入术后听力及语言发育的影响。方法 选择接受人工耳蜗植入术的语言前聋儿童作为研究对象,通过家庭环境量表中文版(Family Environment Scale-Chinese Version, FES-CV)10个量表评价其家庭环境。植入后12个月应用听觉行为分级(categories of auditory performance, CAP)和言语可懂度分级(speech intelligibility rating, SIR)分别对患儿的听力水平和言语水平进行评估。结果 共有60个家庭填写了完整的调查问卷。家庭的亲密度、情感表达、成功性、知识性和娱乐性与CAP得分显著相关,而SIR得分仅与家庭亲密度显著相关。结论 家庭环境与人工耳蜗术后听觉言语发育是相关的。本研究观察到家庭亲密性等多个特征与听力、言语之间的相关性。后续仍然需要进一步的大样本研究来明确其余家庭特征与耳蜗术后康复效果之间的联系。

Objective To evaluate the effects of family environment on hearing and language development in prelingual deafness children after cochlear implantation.Method Prelingually deaf children who received cochlear implantation were selected as the research objects.The Chinese version of the Family Environment Scale was used to evaluate their family environment.The deaf children's linguistic progress was assessed by Categories of Auditory Performance(CAP)and Speech Intelligibility Rating(SIR)at 12 months after implantation.Results A total of 60 families completed the questionnaire.Family cohesion,expressiveness,achievement orientation,intellectual-cultural orientation and active-recreational orientation were significantly related to CAP score,while SIR score was only related to family cohesion.Conclusion Family environment is related to auditory and speech development after cochlear implantation.In this study,we observed the correlation between many characteristics such as family intimacy and listening as well as speech.Further large sample studies are still needed to clarify the relationship between other family characteristics and postoperative hearing rehabilitation.
