合并2型糖尿病冠心病患者的TyG指数与冠状动脉狭窄程度的相关性

Exploring the Relationship between TyG Index and Coronary Artery Stenosis in Patients with Coexisting Coronary Heart Disease and Type 2 Diabetes Mellitus

目的研究血清甘油三酯葡萄糖乘积(TyG)指数与合并2型糖尿病冠心病(CHD)患者冠状动脉狭窄程度的相关性。方法选取2021年2月~2022年2月于承德医学院附属医院住院的127例合并2型糖尿病冠心病患者作为研究对象,依据Gensini评分(≥55分)将其分为高分组(63例)与低分组(64例),采用多因素logistic回归分析TyG指数与冠状动脉狭窄程度的相关性,采用ROC曲线分析TyG指数预测合并2型糖尿病CHD患者冠状动脉狭窄程度的诊断效能。结果多因素logistic回归分析显示,高血压、总胆固醇及TyG指数均是合并2型糖尿病CHD患者冠状动脉狭窄程度的独立危险因素(P<0.05)。TyG指数预测合并2型糖尿病CHD患者冠状动脉狭窄程度的ROC曲线下面积为0.723(95%CI:0.613~0.815,P<0.001),最佳截断值为10.036,灵敏度为71.4%,特异度为73.4%。结论TyG指数是合并2型糖尿病CHD患者冠状动脉狭窄程度的独立危险因素。

Objective To investigate the association between the serum triacylglycerol glucose product(TyG)index and the presence of coronary artery stenosis in patients with coronary heart disease(CHD)combined with type 2 diabetes mellitus(T2DM).Methods A total of 127 cases eligible CHD comlined with type 2 diabetes patients hospitalized at the Affiliated Hospital of Chengde Medical College between February 2021 and February 2022 were included in the study.The CHD combined with T2DM group was categorized into a high score group(63 cases)and a low score group(64 cases)based on the Gensini score(≥55 points).The correlation between the TyG index and the severity of coronary artery stenosis in CHD combined with T2DM patients was analyzed using multivariate logistic regression.Additionally,the diagnostic efficacy of the TyG index in predicting the degree of coronary artery stenosis was assessed using receiver operating characteristic(ROC)curve analysis.Results Multivariate logistic regression analysis revealed that hypertension,total serum cholesterol,and the TyG index were indepondent risk factors for the severity of coronary artery stenosis in patients with CHD combined with T2DM.The area under the curve(AUC)was 0.723(95% CI 0.613-0.815,P<0.001),with an optimal cutoff value of 10.036.The TyG index exhibited a sensitivity of 71.4% and a specificity of 73.4% in predicting the degree of coronary artery stenosis.Conclusion The TyG index emerged as an independent risk factor for the severity of coronary artery stenosis in patients with CHD combined with T2DM.These findings highlight the potential utility of the TyG index as a predictive tool for assessing the degree of coronary artery stenosis in this patient population.
