日间手术模式对经皮肾镜取石术后结石清除率的影响

Influence of day surgery mode on stone removal rate after percutaneous nephrolithotomy

目的探讨日间手术模式对经皮肾镜取石术(PCNL)术后结石清除率的影响。方法回顾性分析2020年7月至2022年3月在粤北人民医院行PCNL治疗的120例肾结石患者的临床资料,其中56例患者接受日间PCNL治疗(日间手术组),64例患者接受常规住院PCNL治疗(住院手术组)。比较两组患者的手术时间、住院时间、医疗费用、结石清除率、疼痛评估结果、满意度及再入院率,统计两组并发症发生率与分级情况。结果日间手术组的住院时间短于住院手术组(P<0.05);医疗费用少于住院手术组(P<0.05);两组手术时间、结石清除率比较,差异均无统计学意义(均P>0.05)。两组疼痛评估结果、并发症发生率与分级情况、患者满意度比较,差异均无统计学意义(P>0.05)。两组患者再入院率比较,差异无统计学意义(1.79%vs.3.13%,P=1.000)。结论日间手术模式下实施PCNL治疗安全可行,住院时间短,医疗费用低。

Objective To analyze the influences of day surgery mode on stone removal rate after percutaneous nephrolithotomy(PCNL).Methods A retrospective analysis of the clinical data of 120 patients with kidney stones who underwent PCNL treatment from July 2020 to March 2022 in Yuebei People's Hospital was conducted.Among them,56 patients received day PCNL treatment(day surgery group),and 64 patients received routine inpatient PCNL treatment(inpatient surgery group).The surgery time,hospitalization time,medical expenses,stone removal rate,pain assessment result,satisfaction degree,readmission rate were compared between the two groups,and the incidence of complications and classification were calculated.Results The hospitalization time in the day surgery group was shorter than that in the inpatient surgery group(P<0.05),and the medical expenses were less than in the inpatient surgery group(P<0.05),there was no significant difference in the surgery time and stone removal rate between the two groups(all P>0.05).There was no statistically significant difference in the pain assessment results between the two groups(P>0.05).There was no significant difference in complication rate and classification between the two groups(all P>0.05).There was no statistically significant difference in satisfaction between the two groups(P>0.05).There was no significant difference in the readmission rate between the two groups(1.79%vs.3.13%,P=1.000).Conclusions It is safe and feasible to implement PCNL treatment under the mode of day surgery,the hospital stay is short,and the medical cost is low.
