白细胞介素-6-174G/C基因多态性与原发性肾病综合征风险的关系

Relationship between interleukin-6-174G/C gene polymorphism and risk of primary nephrotic syndrome

摘要 目的探讨白细胞介素-6-174G/C(IL-6-174G/C)基因多态性与原发性肾病综合征(PNS)发生风险的关系。方法选取2019年1月至2022年5月在本院收治的78例PNS患者(PNS组)和50例体检正常者(对照组)。采用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)法检测IL-6-174G/C基因的多态性,并进行二元logistic回归分析,进一步分析IL-6-174G/C基因多态性与实验室指标及PNS病理类型的关系。结果等位基因(G、C)和基因型(GG、GC、CC)的分布频率比较,差异均无统计学意义(均P>0.05)。CC、GC基因型患者的24 h尿蛋白低于GG基因型患者(均P<0.05)。三种基因型患者的白蛋白、尿素、血肌酐、C反应蛋白水平比较,差异均无统计学意义(均P>0.05)。在PNS患者中,微小病变肾病患者的GG基因型明显高于CC和GC基因型(均P<0.05)。系膜增生性肾小球肾炎、膜性增生性肾小球肾炎、膜性肾病和局灶节段性肾小球硬化患者的基因型分布比较,差异均无统计学意义(均P>0.05)。结论IL-6-174G/C基因多态性与PNS发病无关,而IL-6-174G/C基因多态性与24 h尿蛋白水平显著相关,可作为诊断微小病变肾病的有效指标。 Objective To investigate the relationship between interleukin-6-174G/C(IL-6-174G/C)gene polymorphism and the risk of primary nephrotic syndrome(PNS).Methods A total of 78 PNS patients(PNS group)and 50 healthy individuals(control group)admitted to our hospital from January 2019 to May 2022 were selected.The polymerase chain reaction restriction fragment length polymorphism(PCR-RFLP)method was used to detect the polymorphism of IL-6-174G/C gene,and binary logistic regression analysis was performed.The relationship between IL-6-174G/C gene polymorphism and laboratory indexes and pathological types of PNS was analyzed.Results There was no statistically significant difference in the distribution frequency of alleles(G,C)and GG,GC,CC genotypes(all P>0.05).The 24-hour urinary protein level of patients with CC and GC genotype was significantly lower than that of patients with GG genotype(all P<0.05).There was no significant difference in the levels of albumin,urea,blood creatinine,C-reactive protein among the three genotypes(all P>0.05).In PNS patients,GG genotype were significantly higher than CC and GC genotypes in patients with minimal lesion nephropathy(all P<0.05).There was no significant difference in genotype distribution among patients with mesangial proliferative glomerulonephritis,membranous proliferative glomerulonephritis,membranous nephropathy and focal segmental glomerulosclerosis(all P>0.05).Conclusions IL-6-174G/C gene polymorphism was not associated with the incidence of PNS,while IL-6-174G/C gene polymorphism was significantly associated with 24-hour urinary protein level,which may be an effective index for the diagnosis of micro pathological nephropathy.
