腕踝针联合芬太尼在心房颤动导管射频消融术中的镇痛效果

Analgesic Effect of Wrist⁃Ankle Needling Combined with Fentanyl in Radio⁃frequency Catheter Ablation for Atrial Fibrillation

目的:观察腕踝针联合芬太尼在心房颤动(简称房颤)导管射频消融术中的镇痛效果以及对患者焦虑的影响。方法:将行房颤导管射频消融术的患者70例,按照随机数字表法分为观察组和对照组,每组35例。对照组给予常规剂量芬太尼镇痛,观察组给予腕踝针联合芬太尼镇痛。观察两组患者手术中芬太尼使用剂量及镇痛效果。结果:对照组术中镇痛药物总剂量中位数为2000(700)μg·kg^(-1),观察组术中镇痛药物总剂量中位数为185(165)μg·kg^(-1),观察组使用剂量明显少于对照组,差异有统计学意义(P<0.05);两组手术前后焦虑评分比较,差异有统计学意义(P<0.05),组间比较,差异无统计学意义(P>0.05);观察组VAS评分低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率高于对照组,但差异无统计学意义(P>0.05)。结论:腕踝针联合芬太尼在房颤导管射频消融术中可以减少术中镇痛药物的使用总剂量,改善患者焦虑情绪,减轻疼痛。

Objective:To observe the analgesic effect of wrist⁃ankle needling combined with Fentanyl in radio⁃frequency catheter ablation for atrial fibrillation and the influence on anxiety in patients.Methods:A total of 70 patients undergoing radio⁃frequency catheter ablation for atrial fibrillation were divided into the observation group and the control group according to the random number table method,with 35 cases in each group.The control group was given conventional dosage of Fentanyl for analgesia,while the observation group was given wrist⁃ankle needling combined with Fentanyl for analgesia.The dosage and analgesic effect of Fentanyl during operation were observed.Results:The median total dosage of analgesic drugs in the control group was 2000(700)μg·kg^^(-1),and that in the observation group was 185(165)μg·kg^(-1).The dosage in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).There was a statistically significant difference in anxiety scores between the two groups before and after operation(P<0.05),but no statistically significant difference between the two groups(P>0.05).The VAS score of the ob⁃servation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The incidence of ad⁃verse reactions in the observation group was higher than that in the control group,but the difference was not statistically significant(P>0.05).Conclusion:Wrist⁃ankle needling combined with Fentanyl in radio⁃frequency catheter ablation for atrial fibrillation can reduce the total dosage of analgesic drugs used during operation,improve patients′anxiety and relieve pain.
