神经系统副肿瘤综合征合并甲状腺功能减退性神经病变1例报道及文献复习

Paraneoplastic Neurological Syndrome with Hypothyroid Neuropathy in 1 Cases Report and Literature Review

目的探讨神经系统副肿瘤综合征合并甲状腺功能减退性神经病变的临床表现及诊治特点。方法回顾性分析神经系统副肿瘤综合征合并甲状腺功能减退性神经病变患者的临床资料,并结合文献进行分析。结果该患者抗Amphiphysin抗体阳性,合并肺癌、甲状腺功能减退、睡眠呼吸暂停综合征等,治疗上予以免疫球蛋白冲击治疗,补充甲状腺素等治疗,患者症状有改善。结论甲状腺功能减退性神经病变、副肿瘤综合征均属罕见病,预后不佳,对合并神经系统的甲状腺功能减退患者可行相关副肿瘤抗体检测,行甲状腺素替代治疗及免疫球蛋白冲击治疗可改善患者预后。

Objective To investigate the clinical manifestations and characteristics of diagnosis and treatment of paraneoplastic neurological syndrome with hypothyroid neuropathy.Methods 1 cases were retrospectively analyzed for paraneoplastic neurological syndrome with hypothyroid neuropathy in patients with clinical data,combined with literature analysis.Results The patients were positive for anti-amphiphysin antibody,with lung cancer,hypothyroidism,sleep apnea syndrome.In terms of treatment,the patient was treated with immunoglobulin pulse therapy,and thyroxine supplementation.Patients symptoms were alleviated.Conclusion Hypothyroid neuropathy and paraneoplastic syndrome are rare diseases with poor prognosis.It is feasible to detect the related paraneoplastic antibodies in hypothyroid patients with nervous system.Thyroid hormone replacement therapy and immunoglobulin pulse therapy can improve the prognosis of patients.
