呼出气一氧化氮和过敏原与婴幼儿反复喘息的相关性分析

摘要 目的探究呼出气一氧化氮(FeNO)和过敏原与婴幼儿反复喘息的相关性。方法选取2020年1月~2020年12月在开封市人民医院儿科就诊的≤3岁反复喘息婴幼儿150例作为观察组,另选取同期在本院进行诊治的支气管肺炎患儿50例作为对照组,对所有患儿进行FeNO、肺功能以及过敏原检测。结果在喘息性肺炎组,<1岁与1~3岁食入性过敏原中鸡蛋白阳性率存在统计学差异(P<0.05);在支气管哮喘组,<1岁与1~3岁食入性过敏原中牛肉阳性率存在统计学差异(P<0.05)。支气管肺炎、喘息性肺炎、支气管哮喘在食入性过敏原鸡蛋白阳性率中存在统计学差异(P<0.05)。在支气管肺炎组,FeNO水平与鸡蛋白slgE呈正相关(P<0.05);在支气管哮喘组,FeNO水平分别与血清tIgE、鸡蛋白sIgE呈正相关(P<0.05)。结论不同年龄段婴幼儿反复喘息的过敏原情况不同,其中支气管哮喘患儿FeNO水平与血清tlgE水平呈正相关,临床可用血清tlgE水平来衡量气道的嗜酸性炎症情况。 Objective To investigate the correlation between exhaled nitric oxide(FeNO)and allergens and recurrent wheezing in infants and young children.Methods One hundred and fifty cases of infants and children≤3 years old with recurrent wheezing who attended the pediatric department of Kaifeng People's Hospital from January 2020 to December 2020 were selected as the observation group,and another 50 cases of children with bronchopneumonia who were treated in our hospital during the same period were selected as the control group,and FeNO,pulmonary function and allergen tests were performed on all children.Results In the wheezing pneumonia group,there was a statistical difference in the positive rate of egg white among the ingested allergens between children<1 year old and those between 1 and 3 years old(P<0.05);in the bronchial asthma group,there was a statistical difference in the positive rate of beef among the ingested allergens between children<1 year old and those between 1 and 3 years old(P<0.05).In the bronchopneumonia,wheezing pneumonia and bronchial asthma groups,there was a statistical difference(P<0.05)in the rate of eggwhite positivity among the ingested allergens.In the bronchopneumonia group,FeNO levels were positively correlated with egg white sIgE(P<0.05);in the bronchial asthma group,FeNO levels were positively correlated with serum tIgE and egg white sIgE,respectively(P<0.05).Conclusion The allergen levels of recurrent wheezing in infants and young children of different age groups vary,with a positive correlation between FeNO levels and serum tIgE levels in children with bronchial asthma.Serum tIgE levels can be used in clinical practice to measure the eosinophilic inflammation of the airways.
