摘要
目的:分析慢性乙型肝炎病毒(HBV)感染人群乙醛脱氢酶2(ALDH2)基因多态性特点。方法:选取2018年6月—2022年1月庆阳市人民医院传染科肝病门诊就诊的有饮酒习惯且乙肝表面抗原(HBsAg)阳性患者96例为研究对象,其中乙肝e抗原阳性31例。收集患者基本资料及生化指标,通过检测ALDH2基因多态性,分析不同基因分型相关指标及HBV-DNA病毒载量。结果:ALDH2基因多态性有3种,即ALDH2 Glu504Glu(GG)型、ALDH2 Glu504Lys(GL)型、ALDH2 Lys504Lys(LL)型。GG组天门冬氨酸氨基转移酶、丙氨酸氨基转移酶、γ-谷氨酰转移酶、碱性磷酸酶水平高于GL、LL组,差异有统计学意义(P<0.05)。野生型慢性乙型肝炎(CHB)患者HBV-DNA水平高于突变型,差异有统计学意义(P=0.021)。结论:饮酒状况、基因分布与HBV感染者的疾病发展有相关性,提示有饮酒嗜好的CHB患者应及时检测ALDH2基因多态性,从而更好地的指导诊断、早期干预,改善预后。
Objective:To analyze the gene polymorphism characteristics of acetaldehyde dehydrogenase 2(ALDH2)in chronic hepatitis B virus(HBV)-infected population.Methods:A total of 96 patients with a history of alcohol consumption and tested positive for hepatitis B surface antigen(HBsAg)who were admitted to the Liver Disease Outpatient Clinic,Department of Infection,Qingyang People’s Hospital from June 2018 to January 2022 were selected as the study subjects,of whom 31 cases were positive for hepatitis B e antigen.The patients′basic information and biochemical indexes were collected,and the ALDH2 gene polymorphism was assayed,and different genotyping-related indexes and HBV-DNA viral load were analyzed.Results:Three types of ALDH2 gene polymorphisms were identified,ALDH2 Glu504Glu(GG)type,ALDH2 Glu504Lys(GL)type,and ALDH2 Lys504Lys(LL)type.The levels of aspartate aminotransferase,alanine aminotransferase,γ-glutamyl transferase and alkaline phosphatase in the GG group were higher than those in the GL and LL groups,and the differences were statistically significant(P<0.05).The HBV-DNA level in wild-type chronic hepatitis B(CHB)patients was higher than that in mutant-type patients,and the difference was statistically significant(P=0.021).Conclusion:Alcohol consumption status and gene distribution correlate with disease progression in HBV-infected individuals,suggesting that CHB patients with a drinking habit should be tested for ALDH2 gene polymorphisms in a timely manner to better guide diagnosis,early intervention,and improve prognosis.
