不典型川崎病患儿的临床表现及实验室指标分析

Clinical Manifestations and Laboratory Indicator Analysis of Atypical Kawasaki Disease in Children

目的:分析不典型川崎病患儿的临床表现及实验室指标。方法:选取2016年7月—2021年4月成武县人民医院收治的25例不典型川崎病患儿作为观察组,选取同期25例典型川崎病患儿作为对照组。比较两组临床表现及实验室指标。结果:观察组四肢硬肿、多形性皮疹、颈淋巴结肿大发生率低于对照组,发热持续时间短于对照组,差异有统计学意义(P<0.05)。两组球结膜充血、口唇充血发生率比较,差异无统计学意义(P>0.05)。观察组白蛋白、C反应蛋白、血沉、白细胞计数水平及冠状动脉病变发生率高于对照组,差异有统计学意义(P<0.05);两组血红蛋白水平比较,差异无统计学意义(P>0.05)。结论:不典型川崎病患儿的诊断困难,临床医师可通过实验室指标、临床表现综合诊断,以提升诊断准确性,降低漏诊、误诊率,改善患儿预后。

Objective:To analyze the clinical manifestations and laboratory indicators in the children with atypical Kawasaki disease.Methods:From July 2016 to April 2021,25 children with atypical Kawasaki disease who were admitted to Chengwu County People's Hospital were selected as the observation group,and 25 children with typical Kawasaki disease admitted during the same period were selected as the control group.The clinical manifestations and laboratory indicators of the two groups were compared.Results:The incidence of limbs stiffness,pleomorphic rash and neck lymph node enlargement in the observation group was lower than that in the control group,the fever duration in the observation group was shorter than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of bulbar conjunctival hyperemia,mouth and lip hyperemia between two groups(P>0.05).The levels of albumin,C-reactive protein,erythrocyte sedimentation rate,white blood cell count leukocyte and incidence of coronary artery lesions in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in hemoglobin level between two groups(P>0.05).Conclusion:It is difficult to diagnose the children with atypical Kawasaki disease.Clinicians may conduct a comprehensive diagnosis through laboratory indicators and clinical manifestations to improve the accuracy of diagnosis,reduce the rate of missed diagnosis and misdiagnosis,and improve the prognosis of children.
