基于快速康复外科理念下的围术期护理联合针对静脉血栓栓塞症的护理在股骨颈骨折患者中的应用效果

Application Effect of Perioperative Nursing Based on Enhanced Recovery After Surgery Concept Combined with Venous Thromboembolism Nursing in Patients with Femoral Neck Fracture

目的:探讨基于快速康复外科(ERAS)理念下的围术期护理联合针对静脉血栓栓塞症(VTE)的护理在股骨颈骨折患者中的应用效果。方法:选取2020年1月—2021年12月于靖江市人民医院行全髋关节置换术的84例股骨颈骨折患者作为研究对象,将2020年1—12月收治的42例患者作为对照组,2021年1—12月收治的42例患者作为观察组。对照组实施常规围术期护理,观察组实施基于ERAS理念下的围术期护理联合针对VTE的护理。比较两组Harris髋关节功能评分、VTE及其他并发症发生情况。结果:术前,两组Harris髋关节功能评分比较,差异无统计学意义(P>0.05);术后1周、1个月、3个月,观察组Harris髋关节功能评分均高于对照组,差异有统计学意义(P=0.001)。观察组术后3个月内VTE发生率低于对照组,差异有统计学意义(P=0.048)。观察组术后并发症发生率低于对照组,差异有统计学意义(P=0.035)。结论:基于ERAS理念下的围术期护理联合针对VTE的护理在股骨颈骨折患者中的应用效果显著,可改善患者髋关节功能,降低术后VTE等并发症发生风险。

Objective:To explore the application effect of perioperative nursing based on enhanced recovery after surgery(ERAS)concept combined with venous thromboembolism(VTE)nursing in patients with femoral neck fracture.Methods:A total of 84 patients with femoral neck fracture who underwent total hip replacement in Jingjiang People's Hospital from January 2020 to December 2021 were selected as the study subjects.The 42 patients admitted from January 2020 to December 2020 were included as the control group,and the 42 patients admitted from January 2021 to December 2021 were included as the observation group.The control group implemented routine perioperative nursing and the observation group implemented perioperative nursing based on ERAS concept and VTE nursing.Harris score for hip joint function,VTE and other complications were compared between the two groups.Results:Before surgery,there was no significant difference in Harris score for hip joint function between the two groups(P>0.05).At 1 week,1 month and 3 months after surgery,Harris scores for hip joint function in the observation group were higher than those in the control group,and the difference was statistically significant(P=0.001).The incidence of VTE in the observation group within 3 months after surgery was lower than that in the control group,and the difference was statistically significant(P=0.048).The incidence of postoperative complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.035).Conclusion:The perioperative nursing based on ERAS concept combined with VTE nursing has a significant application effect in patients with femoral neck fracture,it can improve the hip joint function and reduce the occurrence risk of postoperative complications such as VTE.
