基于DRGs付费方式的重症医学科亏损情况分析

Analysis of Deficit of Intensive Care Medicine Department Based on DRGs Payment Method

目的:分析2021—2022年桂林市人民医院重症医学科基于DRGs付费方式亏损情况,探讨控制亏损的可行对策。方法:描述性统计分析2021—2022年桂林市人民医院重症医学科的亏损情况。统计分析DRGs付费方式下亏损病例人次和费用,采用Excel表格汇总2021—2022年所有重症医学科病例的费用信息,将亏损额度变动和各项目费用占比情况作为亏损分析的数据支持。结果:2021—2022年总亏损呈下降趋势,两年间费用信息组成情况中,药费、材料费占比最高;平均住院总费用、药费和材料费均有所下降。结论:DRGs付费方式下,桂林市人民医院可以通过提高医务人员对DRGs的认识、加强对药品和耗材的管控、提高医务人员的编码能力等措施减少亏损。

Objective:To analyze the loss of intensive care medicine department based on DRGs payment in Guilin People's Hospital from 2021 to 2022,and explore the feasible countermeasures to control the loss.Methods:Descriptive statistics were used to analyze the deficit of intensive care medicine department in Guilin People's Hospital from 2021 to 2022.The number and cost of loss cases under DRGs payment were analyzed statistically.Excel tables were used to summarize the cost information of all cases in intensive care medicine departments from 2021 to 2022,and the changes in the amount of loss and the proportion of costs of each item were used as data support for loss analysis.Results:Total loss from 2021 to 2022 showed a downward trend,and drug and material costs accounted for the highest proportion in the two-year cost information;the average total hospitalization expenses,drug and material costs all decreased.Conclusion:under the DRGs payment mode,guilin People's Hospital can reduce the losses by improving the awareness of medical staff on DRGs,strengthening the management and control of drugs and consumables,and improving the coding capacity of medical staff.
