评估妊娠期高血压疾病凝血功能状态多项检测指标的应用进展

Progress in Application of Multiple Detection Indicators for Assessing Coagulation Function Status in Hypertensive Disorders of Pregnancy

生理状况下,机体的止血与抗凝作用处于动态平衡。但在妊娠期高血压疾病(hypertensive disorders of pregnancy,HDP)病理状况下,由于全身小血管痉挛和血管内皮损伤,这种平衡被打破,体现为血小板和凝血因子被激活,抗凝物质释放减少、抗纤溶物质分泌增加等,最终导致血栓形成甚至引发弥散性血管内凝血(disseminated intravascular coagulation,DIC)。因此,应用便捷、高效的检测指标准确评估HDP凝血功能状态,对指导临床进行适时干预、减少孕妇并发症出现、改善孕妇与胎儿结局具有重要意义。对近年来用于评估HDP凝血功能状态的检测指标的应用进展进行简要综述,以期为HDP的血液学检查提供更多实践依据。

Under physiological condition,hemostasis and anticoagulation are in dynamic balance.However,in the pathological condition of hypertensive disorders of pregnancy(HDP),due to systemic spasm of small vessels and vascular endothelial injury,this balance is broken,which is reflected in activation of platelets and coagulation factors,decreased release of anticoagulants and increased secretion of antifibrinolytic substances,etc.,and ultimately leads to thrombosis and even disseminated intravascular coagulation(DIC).Therefore,the application of convenient and efficient detection items to accurately evaluate the coagulation function of HDP is of great significance for guiding timely clinical intervention,reducing the occurrence of complications in pregnant women,and improving the outcomes of pregnant women and fetuses.In order to provide more practical basis for HDP hematology test,the application progress of indicators used to evaluate HDP coagulation status in recent years are briefly reviewed.
