自体毛囊单位提取技术在男性雄激素性脱发中的应用效果

Clinical Application of Follicular Unit Extraction in Androgenetic Alopecia

目的 探讨自体毛囊单位提取(follicular unit extraction, FUE)毛发移植术治疗男性雄激素性脱发(androgenetic alopecia, AGA)的临床效果。方法 本研究为回顾性分析,研究对象为2016年8月—2018年9月西安医学院第一附属医院整形烧伤美容科行FUE毛发移植术的男性AGA患者。移植术后1年时,测量受区存活的毛发密度并计算毛囊存活率,以评估临床疗效;记录移植术后并发症发生情况、患者满意度及移植前后生活质量评分。结果 共入选符合纳入与排除标准的男性AGA患者126例,平均年龄为(37±9.05)岁;平均病程为(10±2.76)年;Hamilton-NorwoodⅡ级5例,Ⅲ级55例,Ⅳ级54例,Ⅴ级10例,Ⅵ级2例。移植术后1年时,前发际、头顶处存活的毛发密度分别为(65±7)FUs/cm^(2)、(43±4)FUs/cm^(2);毛囊存活率均>90%(其中107例患者毛囊存活率>95%),有效率为100%(126/126);均无严重并发症发生,患者满意率为97.6%(123/126)。移植术后1年时,126例AGA患者生活质量综合评定量表-74中的躯体功能、心理功能、社会功能和物质生活状态评分均较术前明显提高(P均<0.01)。结论 FUE毛发移植术具有安全性好、毛囊移植后存活率高的特点,治疗男性AGA有效率高并可提高生活质量,患者满意度高。

Objective To explore the clinical application of follicular unit extraction(FUE)hair trans⁃plantation in male androgen alopecia(AGA).Methods From August 2016 to September 2018,male AGA patients operated with FUE hair transplantation in the First Affiliated Hospital of Xi􀆳an Medical University were retrospectively analyzed.At one year after transplantation,clinical curative effect was evaluated by the survival hair density and follicles rate of FUE transplantation,meanwhile surgical complications,patient satisfaction rate and life quality score were measured and analyzed.Results A total of 126 male AGA patients who met the in⁃clusion and exclusion criteria were included,with average age was(37±9.05)years and average disease course was(10±2.76)years.According to the Hamilton⁃Norwood grade standard,there were 5 gradeⅡpatients,55 gradeⅢpatients,54 gradeⅣpatients,10 gradeⅤpatients,and 2 gradeⅥpatients.At one year after transplantation,viable hair density on the anterior hairline and the top of the head was(65±7)FUs/cm^(2) and(43±4)FUs/cm^(2),respectively.The survival rate of hair follicles was more than 90%,even more than 95%among the 107 AGA patients.The effective rate was 100%(126/126),no serious complications occurred,and the satisfaction rate of patients was 97.6%(123/126).Besides,the scores of physical function,psychological function,social function and material life status of 126 AGA patients were significantly increased than those before transplantation(all P<0.01).Conclusions FUE hair transplanta⁃tion has significant clinical effect,and is highly safe and effective.With the potential to dramatically improve life quality,it can be widely applied in treatment of AGA.
