择时穴位按摩对中风后偏瘫患者心理弹性、日常生活能力及自我效能评分的影响

Effect of timing acupoint massage on resilience,daily living ability and self-efficacy in patient with hemiplegia after stroke

[目的]分析择时穴位按摩对中风后偏瘫患者心理弹性、日常生活能力及自我效能评分的影响。[方法]选取2021年6月—2022年6月首都医科大学附属北京中医医院收治的中风后偏瘫患者120例,采用随机数字表法分为对照组和观察组,各60例。对照组接受常规护理,观察组接在常规护理基础上接受择时穴位按摩,7 d为1周期,干预3周。比较干预前、干预3周后心理弹性、日常生活能力、自我效能、运动功能、神经缺损及步态,干预3周后患者的护理满意度。[结果]与干预前比较,干预3周后,两组心理弹性量表(CD-RISC)、改良Barthel指数(MBI)、一般自我效能感量表(GESE)、Fugl-Meyer量表(FMA)、Holden步行功能分级(FAC)评分,步长、步幅、步速均升高,且观察组高于对照组(P<0.05)。与干预前比较,干预3周后,两组美国国立卫生研究院卒中量表(NIHSS)评分均降低,且观察组低于对照组(P<0.05)。干预3周后,观察组患者对病情介绍、护患沟通、护理能力以及护理态度的满意度评分更高(P<0.05)。[结论]择时穴位按摩可改善中风后偏瘫患者心理弹性、日常生活能力及自我效能,提高运动功能,调节患者步态,降低神经缺损,取得患者认可。

[Objective]To analyze the effect of timing acupoint massage on the psychological resilience,daily living ability and self-efficacy of patients with hemiplegia after stroke.[Methods]From June 2021 to June 2022,120 patients with hemiplegia after stroke admitted to Beijing Hospital of Traditional Chinese Medicine,Capital Medical University were divided into the control group and the observation group,with 60 cases in each group by random number table method.The control group received routine nursing,and the observation group received timing acupoint massage on the basis of routine nursing,7 d as a cycle,intervention for 3 weeks.The psychological resilience,daily living ability,self-efficacy,motor function,nerve defect,gait before intervention,after 3 weeks of intervention,and nursing satisfaction of patients after 3 weeks of intervention were compared in the two groups.[Results]Compared with before intervention,after 3 weeks of intervention,the scores of Connor-Davidson Resilience Scale(CD-RISC),Modified Barthel Index(MBI),General Self-efficacy Scale(GESE),Fugl-Meyer Scale(FMA),functional ambulation classification(FAC),step length,stride length and pace of the two groups all increased,and the observation group was higher(P<0.05).Compared with before intervention,after 3 weeks of intervention,the score of National Institutes of Health Stroke Scale(NIHSS)of the two groups decreased,and the score of the observation group was lower(P<0.05).After 3 weeks of intervention,the scores of satisfaction of patients in the observation group on condition introduction,nurse-patient communication,nursing ability and nursing attitude were higher(P<0.05).[Conclusion]Timing acupoint massage could improve the psychological resilience,daily living ability and self-efficacy of patients with hemiplegia after stroke,improve motor function,adjust the gait of patients,reduce nerve defects,and obtain patients’recognition.
