慢性阻塞性肺疾病合并冠心病患者可溶性晚期糖基化终产物受体的初步研究

摘要 目的 探讨慢性阻塞性肺疾病(COPD)合并冠心病患者可溶性晚期糖基化终产物受体(sRAGE)的变化情况及临床意义。方法 选取2018年1月—2020年1月北京市垂杨柳医院呼吸科病房收治的COPD合并冠心病患者40例(合并冠心病组),同时选取呼吸科病房收治的COPD患者40例(单纯COPD组),体检健康者30例(对照组)。收集所有研究对象基线资料,比较三组患者的肺功能及sRAGE指标的差异。结果 合并冠心病组和单纯COPD组血浆sRAGE水平、FEV_(1)%显著低于对照组(P均<0.05)。合并冠心病组血浆sRAGE水平、FEV_(1)%与单纯COPD组相比明显下降(P均<0.05)。合并冠心病组sRAGE水平与FEV_(1)%呈正相关(r值为0.637,P<0.05)。结论 sRAGE在COPD合并冠心病患者血浆中呈低表达,其有可能反映COPD合并冠心病的患者的肺功能损害程度,对疾病的严重程度判别有一定的帮助。 Objective To investigate the changes and clinical significance of soluble advanced glycation end product receptor(sRAGE)in blood of patients with chronic obstructive pulmonary disease(COPD)complicated with coronary heart disease.Methods From January 2018 to January 2020,40 patients with COPD complicated with coronary heart disease(the combined with coronary heart disease group)were selected from the respiratory ward of Beijing Chuiyangliu hospital,40 patients with COPD(the simple COPD group)and 30 healthy physical examination patients at the same age(the control group)were selected.The baseline data of all subjects were collected,and the differences of pulmonary function and sRAGE indexes among the three groups were compared.Results The plasma sRAGE level and FEV_(1)%in the combined coronary heart disease group and the simple COPD group were significantly lower than those in the control group(P<0.05).The plasma sRAGE level and FEV_(1)%in the combined coronary heart disease group were significantly lower than those in the simple COPD group(P<0.05).sRAGE was positively correlated with FEV_(1)%in the group with coronary heart disease(r=0.637,P<0.05).Conclusion sRAGE is low expressed in the plasma of COPD patients complicated with coronary heart disease,which may reflect the degree of pulmonary function damage in COPD patients with coronary heart disease,and is helpful to judge the severity of the disease.
