维持性血液透析患者硫酸对甲酚水平与微炎症、氧化应激的关系 被引量:1

Relationships of p-Cresyl Sulfate Levels to Microinflammation and Oxidative Stress in Maintenance Hemodialysis Patients

摘要 目的探讨维持性血液透析(MHD)患者的硫酸对甲酚(PCS)水平与微炎症、氧化应激的关系.方法选取2021-2022年门诊和住院部收治的MHD患者90例(MHD组),另择取同期健康体检者30例纳入健康对照组(健康组),比较2组血清血白蛋白(Alb)、视黄醇结合蛋白(RBP)、铁蛋白(SF)、微炎症相关标志物[C-反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、氧化应激指标[谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)]和血浆PCS水平.采用Pearson相关性分析血浆PCS含量与血清Alb、RBP、SF、微炎症相关标志物、氧化应激指标水平的相关性;并建立多因素Logistic回归分析模型,通过多元逐步回归分析寻找早期预测指标.结果MHD组的血清Alb、GSH-PX、SOD水平低于健康组(P<0.05),MHD组血清RBP、SF、hs-CRP、IL-6及TNF-α水平和血浆PCS水平高于健康组(P<0.05);Pearson相关性分析结果显示,MHD患者血浆PCS水平与血清RBP、SF、hs-CRP、IL-6、TNF-α水平呈正相关(r=0.717、0.682、0.580、0.724、0.579,均P<0.05),与血清Alb、GSH-PX、SOD水平呈负相关(r=-0.551、-0.533、-0.822,均P<0.05);多因素Logistics逐步回归分析结果显示,血浆PCS水平与血清Alb、GSH-PX、SOD水平呈负相关(β=-2.847、-2.956、-3.212,均P<0.05),血浆PCS水平与血清RBP、SF、hs-CRP、IL-6、TNF-α水平呈正相关(β=0.072、0.065、0.058、0.078、0.055,均P<0.05).结论MHD患者血清血浆PCS水平升高,且与微炎症和氧化应激有关. Objective To explore the relationships of p-cresol sulfate(PCS)to microinflammation and oxidative stress in maintenance hemodialysis(MHD)patients.Methods Ninety MHD patients admitted to the outpatient and inpatient departments from 2021-2022 were selected in this study.In addition,30 healthy individuals who underwent physical examination during the same period were selected as the control group.Serum levels of albumin(Alb),retinol binding protein(RBP),ferritin(SF),high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),glutathione peroxidase(GSH-Px)and superoxide dismutase(SOD)and plasma PCS content were compared between the two groups.Pearson correlation analysis was used to investigate the relationships of plasma PCS content to serum Alb,RBP and SF levels,microinflammatory markers,and oxidative stress indicators.Multivariate logistic regression analysis model was established to search for early predictive indicators by multiple stepwise regression analysis.Results Compared with the healthy group,serum Alb,GSH-PX and SOD levels decreased,and serum RBP,SF,hs-CRP,IL-6 and TNF-αlevels increased in the MHD group(P<0.05).Pearson correlation analysis showed that plasma PCS content in MHD patients was positively correlated with serum RBP,SF,hs-CRP,IL-6 and TNF-αlevels(r=0.717,0.682,0.580,0.724 and 0.579,respectively;P<0.05),but negatively associated with serum Alb,GSH-PX and SOD levels(r=-0.551,-0.533 and-0.822,respectively;P<0.05).Multivariate logistic stepwise regression analysis showed that plasma PCS content was negatively related to serum Alb,GSH-PX and SOD levels(β=-2.847,-2.956 and-3.212,respectively;P<0.05),but positively correlated with serum RBP,SF,hs-CRP,IL-6 and TNF-αlevels(β=0.072,0.065,0.058,0.078 and 0.055,respectively;P<0.05).Conclusion Plasma PCS levels increase in MHD patients and are associated with microinflammation and oxidative stress.
