基于早年饥荒暴露与成年代谢综合征患病风险在暴露时间及地区层面的横断面关联研究

A cross-sectional study of early childhood famine exposure and adult meta bolic syndrome risk at the time of exposure and regional level

目的:通过流行病学调查,评估生命早期暴露于饥荒在暴露时间、地区差异方面与成年代谢综合征(Metabolic Syndromes,MetS)患病率之间的关系。方法:利用2018年“中国健康与营养调查”项目(CHNS)数据库,采用横断面研究设计,通过分层、随机整群抽样抽取3454名研究对象,根据中华医学会糖尿病学分会(CDS)的诊断标准及经历饥荒的时期将研究对象分为5组,初步评价早年不同时期饥荒暴露时间、地区与成年MetS之间的关系。结果:早年时期暴露于饥荒与成年MetS的患病风险呈正相关(P=0.0002)。城市地区人群中少年期暴露于饥荒可增加成年MetS的患病风险(P=0.0014);婴儿期暴露于饥荒可提高成年MetS的患病风险(P=0.0195)。结论:早年时期暴露于饥荒与成年MetS的患病风险密切相关。对慢性非传染性疾病及早筛查、精准干预并增强早期发育阶段的营养可降低MetS的发病率。

Objective:To evaluate the association of early life exposure to famine with the prevalence of adult Metabolic Syndromes(MetS)in terms of exposure time,regional differences through epidemiological investigation.How:Using the 2018 China Health and Nutrition Survey(CHNS)database,a cross-sectional study design was adopted to select 3454 subjects through stratified and random cluster sampling,and the subjects were divided into five groups according to the diagnostic criteria of Diabetes Society of Chinese Medical Association(CDS)and the period of famine.The relationship between famine exposure time,region and adult MetS in different early years was evaluated.Results:Early exposure to famine was positively associated with the risk of adult MetS(P=0.0002).Adolescent exposure to famine in urban areas increased the risk of adult MetS(P=0.0014).Exposure to famine in infancy increased the risk of adult MetS(P=0.0195).Conclusions:Exposure to famine in early life is strongly associated with the risk of adult MetS.Early screening for chronic noncommunicable diseases,targeted interventions,and enhanced nutrition in early development can reduce the incidence of MetS.
