摘要
目的:探讨尿液中性粒细胞明胶酶相关脂质运载蛋白(NGAL)与肾小管上皮细胞(TEC)联合检测在肾小管损伤早期诊断中的价值。方法:将2020年1月至10月入院的肾脏病患者110例纳入病例组,选取同期健康体检者100例纳入对照组。检测两组尿液中的TEC数量及尿NGAL水平,采用Spearman相关性分析法分析两者之间的相关性,统计TEC、NGAL单独及联合检测诊断肾小管损伤的敏感度、特异度。结果:病例组尿液中的TEC数量及尿NGAL水平均高于对照组,差异有统计学意义(P<0.05)。经Pearson相关性分析显示,尿液中的TEC数量与尿NGAL水平呈正相关(r=0.773,P<0.05),当尿液中TEC数量增多时,尿NGAL水平也相应增高。NGAL、TEC单独检测诊断肾小管损伤的敏感度、特异度相比,差异无统计学意义(P>0.05)。TEC与NGAL联合检测诊断肾小管损伤的敏感度、特异度均高于二者单独检测,差异有统计学意义(P<0.05)。结论:TEC、NGAL是反映肾小管损伤的敏感指标,两项指标联合检测诊断肾小管损伤的效能优于二者单独检测,对识别早期肾小管损伤具有一定的意义。
Objective:To investigate the value of urine neutrophil gelatinase-associated lipid carrier protein(NGAL)combined with renal tubular epithelial cells(TEC)in the early diagnosis of renal tubular injury.Methods:A total of 110 patients with kidney disease admitted from January to October 2020 were included in the case group and 100 healthy subjects were included in the control group.The amount of TEC in urine and urinary NGAL level in the two groups were detected,and Spearman correlation analysis was used to analyze the correlation between the two groups,and the sensitivity and specificity of TEC and NGAL alone and combined detection in the diagnosis of renal tubule injury were analyzed.Results:The amount of TEC in urine and the level of NGAL in urine in case group were higher than those in control group,the difference was statistically significant(P<0.05).Pearson correlation analysis showed that the amount of TEC in urine was positively correlated with the level of NGAL in urine(r=0.773,P<0.05).When the amount of TEC in urine increased,the level of NGAL in urine also increased.There was no significant difference in sensitivity and specificity of NGAL and TEC in the diagnosis of renal tubular injury(P>0.05).The sensitivity and specificity of TEC and NGAL combined detection in the diagnosis of renal tubular injury were higher than those of TEC and NGAL alone,and the difference was statistically significant(P<0.05).Conclusion:TEC and NGAL are sensitive indicators of renal tubular injury,and the combined detection of TEC and NGAL is more effective than the single detection of TEC and NGAL in the diagnosis of renal tubular injury,which has certain significance for the identification of early renal tubular injury.
