《结直肠癌化疗期中医诊疗指南》解读

Interpretation of the Guidelines for Traditional Chinese Medicine Diagnosis and Treatment of Colorectal Cancer during Chemotherapy and Sharing of Typical Cases

《结直肠癌化疗期中医诊疗指南》于2020年12月1日在中华中医药学会立项,由全国13家单位18名专家起草,2022年7月正式在中华中医药学会发布。本指南通过德尔菲法确定了5个临床问题并通过循证医学进行了解答。结直肠癌是常见的消化道恶性肿瘤,化疗是其主要治疗手段。然而化疗的不良反应限制了其应用。中医药已被证实可降低化疗期药物不良反应的发生率,还可提高化疗完成率。《结直肠癌化疗期中医诊疗指南》项目组基于临床医师的困惑,开展了专家访谈、问卷调查、文献检索、专家论证等一系列工作,最终形成了《结直肠癌化疗期中医诊疗指南》。目前《结直肠癌化疗期中医诊疗指南》(以下简称《指南》)已正式发布。本文紧密围绕该《指南》撰写的背景、意义、核心内容进行了详细解读,并通过1例病例评价指南的一致性及实用性。希望通过中医药的干预使更多结直肠癌患者从化疗中获益,提高化疗完成率,从而实现结直肠癌患者的长期生存。由本指南制订相关专家及参与工作的医学生组成的研究团队对该指南进行解读。参与指南解读的人员不存在与原指南相关的学术或商业利益冲突。

The Guidelines for Traditional Chinese Medicine Diagnosis and Treatment of Colorectal Cancer during Chemotherapy was initiated on December 1,2020 by the China Association of Chinese Medicine,drafted by 18 experts from 13 organizations nationwide,and officially released in July 2022 by the China Association of Chinese Medicine.The Guidelines has identified 5 clinical questions through the Delphi method and answered them via evidence-based medicine.Colorectal cancer,a common malignant tumor of the digestive tract,is mainly treated by chemotherapy.However,the adverse reactions limit the application of chemotherapy.Traditional Chinese medicine(TCM)has been proved effective in reducing the incidence of adverse reactions caused by chemotherapy and improving the completion rate of chemotherapy.Considering the confusion of clinicians,the editorial team carried out expert interviews,questionnaires,literature retrieval,and expert argumentation to formulate the Guidelines.This article details the background,significance,and core content of the Guidelines,and evaluates the consistency and practicality of the Guidelines through one case study.We hope that the TCM intervention can help more colorectal cancer patients benefit from chemotherapy,improve the completion rate of chemotherapy,and prolong the survival for colorectal cancer patients.A research team composed of experts and medical students involved in the drafting interprets the Guidelines.The personnel involved in the interpretation do not have any academic or commercial conflicts of interest related to the Guidelines.
