针刺配合二仙汤治疗产后抑郁症临床观察

Acupuncture Combined with Erxian Decoction in the Treatment of Postpartum Depression

目的 观察针刺配合二仙汤治疗产后抑郁症的临床效果及对血清雌激素、孕激素水平的影响。方法 将纳入研究的产后抑郁症患者72例随机分为观察组37例,西药组35例。观察组使用针刺配合二仙汤治疗,西药组口服盐酸马普替林片治疗,共6周。应用爱丁堡产后抑郁量表(EPDS)进行疗效评价并检测患者血清雌激素、孕激素水平。结果 治疗6周后,观察组总有效率为94.6%(35/37),高于西药组的62.9%(22/35),差异有统计学意义(P<0.01)。观察组与西药组治疗后的EPDS评分及雌二醇(E2)和孕酮(P)水平与治疗前比较,差异均有统计学意义(P<0.05);观察组治疗后的EPDS评分及E2和P水平与对照组比较,差异有统计学意义(P<0.05)。结论 针刺配合二仙汤治疗产后抑郁症能够通过调节患者的雌、孕激素水平来改善患者的抑郁症状,其具体的内部机制有待于进一步的研究。

Objective To observe the clinical effect of acupuncture combined with Erxian decoction on postpartum depression and its influence on serum estrogen and progesterone levels.Methods 72 patients with postpartum depression(PPD)included in the study were randomly divided into observation groups of 37 cases and Western medicine group of 37 cases.The observation group was treated with acupuncture combined with Erxian decoction,and the Western medicine group was treated orally with peptiling hydrochloride for a total of 6 weeks.The postpartum depression scale of Edinburgh was used to evaluate the curative effect and test the serum estrogen and progesterone levels of patients.Results After 6 weeks of treatment,the total effective rate of observation group was 94.6%(35/37),which was higher than that of western medicine group(62.9%,22/35),and the difference was statistically significant(P<0.01).The EPDS scores and the levels of estradiol(E2)and progesterone(P)after treatment were significantly different between the observation group and the western medicine group(P<0.05).After treatment,EPDS scores,E2 and P levels in the observation group were significantly different from those in the control group(P<0.05).Conclusion Acupuncture combined with Erxian decoction can improve the depression symptoms of patients by regulating the level of estrogen and progesterone in patients.The specific internal mechanism needs to be further studied.
