胆盐诱导急性胰腺炎大鼠模型中TNF-α、IL-6、相关AMS水平变化的研究

Changes of TNF-α,IL-6 and AMS Levels in Rat Model of Bile Saltinduced Acute Pancreatitis

摘要 目的观察胆盐诱导急性胰腺炎大鼠模型中相关淀粉酶(amylase,AMS)、白细胞介素6(interleukin-6,IL-6)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)指标变化。方法纳入黑龙江省医院实验中心2021年1月—2022年7月的成年大鼠(Wistar)50只,根据随机双盲法分组,分为胆盐诱导急性胰腺炎组(n=26)、假手术组(n=24),胆盐诱导急性胰腺炎组建立胆盐诱导急性胰腺炎动物模型,假手术组予以开腹后对十二指肠、胰腺轻揉,分析两组实验动物在开始不同时间点的胰腺血流量,在操作前、操作后不同时间点检测血清中TNF-α、IL-6指标变化,操作后12 h将大鼠处死,取胰腺组织予以病理检查。结果胆盐诱导急性胰腺炎组实验动物在开始0.5、1、1.5、2、2.5、3 h的胰头部微循环血流量、胰体尾部微循环血量和假手术组相比较低,差异有统计学意义(P<0.05);胆盐诱导急性胰腺炎组大鼠在术后2、4、6、12 h肿瘤坏死因子-α、IL-6指标与假手术组相比较高,差异有统计学意义(P<0.05);胆盐诱导急性胰腺炎组实验后的血清脂肪酶、血清淀粉酶指标分别为(3023.2±780.2)、(1740.7±55.6)U/L,与假手术组的(86.6±27.2)、(148.7±37.1)U/L相比较高,差异有统计学意义(t=18.414、118.052,P<0.05);胆盐诱导急性胰腺炎组与假手术组相比,胰腺病理学中性粒细胞浸润、坏死、出血、水肿评分大幅度增高,差异有统计学意义(P<0.05)。结论急性重症胰腺炎患者胰腺区域血流量大幅度降低,产生微循环障碍,炎性因子指标具有差异性。 Objective To observe the changes of related amylase(AMS),interleukin(IL-6),and Tumor Necrosis Factor Alpha(TNF-α)indexes in bile salt-induced acute pancreatitis rat models.Methods Fifty adult rats(Wistar)in Heilongjiang Provincial Hospital Experimental Center from January 2021 to July 2022 were enrolled and randomly divided into bile salt-induced acute pancreatitis group(n=26)and sham operation group(n=24)according to a doubleblind method.Animal models of bile salt-induced acute pancreatitis were established in the bile salt-induced acute pancreatitis group,and the duodenum and pancreas were lightly rubbed in the sham operation group after laparotomy.The pancreatic blood flow of the two groups was analyzed at different time points at the beginning,and the serum TNF-αand IL-6 indexes were detected at different time points before and after the operation.The rats were killed 12 hours after operation,and the pancreatic tissues were taken for pathological examination.Results Compared with the sham operation group,the microcirculation blood flow in the head of the pancreas and the tail of the pancreas body in the bile salt-induced acute pancreatitis group decreased at the beginning of 0.5 h,1 h,1.5 h,2 h,2.5 h and 3 h,the difference was statistically significant(P<0.05).The tumor necrosis factor-αand IL-6 indexes of bile salt-induced acute pancreatitis group were significantly increased at 2 h,4 h,6 h and 12 h after surgery compared with sham operation group,the difference was statistically significant(P<0.05).Serum lipase and serum amylase indexes in the bile salt-induced acute pancreatitis group were(3023.2±780.2)U/L and(1740.7±55.6)U/L,respectively,significantly increased compared with those in the sham operation group(86.6±27.2)U/L and(148.7±37.1)U/L,the difference was statistically significant(t=18.414,118.052,P<0.05).The scores of neutrophil infiltration,necrosis,hemorrhage and edema in the bile salt-induced acute pancreatitis group were higher than those in the sham operation group,the difference was statistically significant(P<0.05).Conclusion In severe acute pancreatitis,the blood flow in the pancreatic region is greatly reduced,resulting in microcirculation disturbance,and inflammatory factor indexes are different.
