急性缺血性脑卒中抽吸导管直接取栓与支架加中间导管取栓疗效的对比研究

Comparison of Efficacy of Direct Thrombectomy with Stent and Intermediate Catheter in Acute Ischemic Stroke

目的探讨急性颅内大动脉闭塞的抽吸导管直接取栓和利用支架加中间导管取栓的诊疗效果,为临床诊疗提供科学参考依据。方法选取2020年1月—2022年10月来齐齐哈尔医学院附属第三医院治疗的急性颅内大动脉闭塞性缺血性卒中患者80例,随机分为对照组与观察组,各40例患者。对照组患者治疗采用取栓支架加中间导管取栓完成,观察组患者治疗采用抽吸导管直接取栓完成,观察其临床治疗疗效。结果观察组手术后14 d的NIHSS评分为(7.71±3.51)分,显著低于手术后7 d的(9.78±2.19)分,手术后14 d的MRS评分为(1.09±0.69)分,均优于对照组,差异有统计学意义(P<0.05)。观察组患者手术后的血管再通率为87.5%,高于对照组的67.5%,差异有统计学意义(χ^(2)=4.588,P<0.05)。结论急性缺血性脑卒中患者采用导管抽吸取栓治疗术临床观察效果显著,大幅度提高了患者堵塞血管的再通率,提高患者治疗效果,预后效果理想。

Objective To explore the diagnosis and treatment effects of direct thrombectomy with suction catheter and thrombectomy with stent and intermediate catheter in acute intracranial great artery occlusion,and to provide scientific reference for clinical diagnosis and treatment.Methods 80 patients with acute intracranial great artery occlusive ischemic stroke treated in the Third Affiliated Hospital of Qiqihar Medical College from January 2020 to October 2022 were selected and randomly divided into control group and observation group,with 40 patients in each group.The control group was treated with thrombectomy stent and intermediate catheter,and the observation group was treated with direct thrombectomy with suction catheter.The clinical therapeutic effect was observed.Results The NIHSS score at 14 days after surgery was(7.71±3.51)points,significantly lower than 7 days after surgery(9.78±2.19)points,and MRS score at 14 days after surgery was(1.09±0.69)points,all were better than those in the control group,the difference was statistically significant(P<0.05).The vascular recirculation rate was 87.5%in the observation group,higher than 67.5%in the control group,the difference was statistically significant(χ^(2)=4.588,P<0.05).Conclusion The clinical observation effect of suction catheter and thrombolysis in patients with acute ischemic stroke was remarkable,which greatly improved the recurrence rate of blocked blood vessels in patients,improved the therapeutic effect of patients,and the prognosis effect was ideal.
